Overview
The Healthcare Services Coordinator supports the Healthcare Services team by providing administrative and reception support. Reporting to the manager of Health Care Services, the coordinator supports youth and staff administrative support.
We are currently recruiting for 2 Part Time, Permanent Health Care Services Coordinators. This is a replacement role within the organization.
Responsibilities:
Welcoming patients as they arrive, ensuring they feel comfortable and attended to. Direct patients to appropriate rooms
Coordinates clinic flow, liaise with staff to promote effective use of services and space and collaborate with team to improve workflows and outcomes for young people
Coordinates appointments and ensures that youth understand details about time, location and cancellation procedure
Facilitates appointments with external clinicians as requested by team
Supports triaging of young people as needed and directed by nursing team or Manager
Notifies the laboratory about pick-ups and deliveries
Coordinates case consults with CHT staff and medical partners
Educate and support patients around clinic processes
Connect young people with community resources, support services, and external healthcare providers as needed
Collect and update patient information, including demographic data and insurance details
Ensure confidentiality and compliance with privacy regulations (i.e. PHIPA)
Process requests for medical records from patients, other healthcare providers or authorized third parties, coordinate follow-up appointments and handle patient reminders
Maintains current knowledge of community services, resources, programs, procedures and issues affecting client population by creating resources binder and community board
Ensure proper release and/or exchange of information forms are completed and complies with regulations
Track and follow up on medical record requests and specialist/outpatient referrals submitted by the clinic, ensuring timely processing and completion
Assist with opening and closing procedures (i.e. daily clinic board)
Respond to emergencies involving clients by assessing the situation and implement solutions.
Ability to prioritize client care based on urgency, safety, and client specific needs
Develop and maintain relationship with community stakeholders and organizations in order to communicate program information and schedule appointments for clients
Answer telephones, schedule appointments, respond to inquiries, follow up on voicemails, handle patient check-in and check-out processes, and handle incoming mails, faxes, or emails and distribute them appropriately.
File, organize and maintain physical or electronic medical records accurately
Ensures the safety and security of health care records and maintains strict confidentiality about all department and youth specific information
Maintenance of schedules and petty cash
Conducts ETO intake process when young people first visit the clinic
Establish and maintain effective communication and working relationships with all patients, Nurse Practitioners, staff and external vendors
Assist patients with completing paperwork at the clinic to avoid errors and inaccurate information
Order supplies as needed from vendors to ensure offices and clinics are stocked
Compiles and maintains statistics on youth specific health care statistics
Compile and enter data on health-related outcomes used for policy and planning and prepare statistical reports using various computer systems
Other additional duties as assigned
Qualifications:
College diploma or university degree in a relevant discipline, such as Health Care Administration.
Minimum 2 years of relevant experience
Experience working with and providing clinically informed services to youth at risk of, or experiencing, homelessness or trafficking, supporting an excellent understanding of issues related to youth homelessness, trafficking, 2SLGBTQ+ identities, trauma, mental health and substance use problems and disorders (and harm reduction), and developmental and learning disabilities.
Sound understanding of primary care, mental health, substance use, and wellness interventions
Strong organizational and time management skills.
Excellent written, documentation and oral communication skills.
Strong computer skills.
Ability to work independently, proactively and collaboratively.
Knowledge of community resources and systems relevant to youth homelessness
Training in UMAB, BLS, First Aid, and CPR.
