We are seeking a compassionate and dedicated Health Care Aide to provide high-quality patient care in various settings, including assisted living, nursing homes, and home health environments. The ideal candidate will have experience working with seniors, individuals with developmental disabilities, or those requiring memory care. This role involves supporting patients with daily activities, monitoring health status, and ensuring their comfort and safety while maintaining strict confidentiality and adhering to healthcare regulations such as HIPAA. Duties
Observe and monitor patients' conditions, including vital signs and behavioral changes
Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
Operate Electronic Medical Records (EMR) and Electronic Health Records (EHR) systems for accurate documentation of patient care
Administer medications and assist with medication management following prescribed protocols
Support patients with dementia or Alzheimer's through specialized memory care techniques
Perform catheter care, wound care, and assist with hospice or palliative needs as required
Use assistive devices such as Hoyer lifts to ensure safe patient transfers
Prepare meals and assist with meal planning in accordance with dietary restrictions
Maintain cleanliness of patient areas, including laundry and general housekeeping duties
Document medical records accurately while respecting patient privacy under HIPAA guidelines
Collaborate with healthcare teams to ensure comprehensive patient care plans are followed effectively
Qualifications
Proven experience as a Health Care Aide or in a similar caregiving role within long-term care, home health, or hospice settings
Knowledge of medical terminology, medication administration, and vital signs measurement
Certification in CPR and first aid is required; additional certifications such as catheter care or dementia training are a plus
Experience working with people with disabilities or developmental challenges preferred
Familiarity with EMR/EHR systems for documentation purposes
Compassionate attitude with excellent communication skills and patience when working with vulnerable populations
Ability to work independently and as part of a multidisciplinary team in a fast-paced environment
Valid driver's license is advantageous for home & community care roles
