We are seeking compassionate and dedicated Health Care Aides to provide exceptional patient-centered care in various settings, including assisted living, nursing homes, and home health environments. The ideal candidate will be responsible for supporting patients with daily activities, ensuring their comfort, safety, and well-being. Experience with medical documentation, EMR/EHR systems, and specialized care such as dementia or Alzheimer's is highly valued. Responsibilities
Monitor and observe patients' conditions, including vital signs and overall health status
Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
Operate medical equipment safely, including Hoyer lifts and catheters
Administer medications accurately following prescribed protocols and document all medication administration in medical records or EMR/EHR systems
Support patients with dementia or Alzheimer's care, memory care, and hospice needs
Assist with meal preparation, feeding, and nutritional needs while adhering to dietary restrictions
Perform light housekeeping duties such as laundry and cleaning to maintain a safe environment
Document patient information accurately in medical records while maintaining HIPAA compliance
Work collaboratively with healthcare professionals to coordinate patient care plans
Support patients during transfers using appropriate assistive devices like Hoyer lifts
Recognize signs of medical emergencies and respond appropriately
Qualifications
Prior experience in senior care, home health, long-term care, or nursing home environments preferred
Knowledge of medical terminology and medical documentation practices
Certification in CPR is required; additional certifications such as medication administration or hospice care are advantageous
Experience working with people with developmental disabilities or dementia is preferred
Familiarity with EMR/EHR systems and HIPAA regulations
Ability to work compassionately with diverse patient populations including those with Alzheimer's disease or other memory impairments
Strong communication skills and ability to work effectively within a healthcare team
Physical ability to assist with patient mobility and perform tasks requiring standing, lifting, or bending
