Observe and monitor residents' health status regularly, noting any changes in condition or behavior.
Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
Administer medications accurately following medical documentation protocols and HIPAA privacy regulations.
Support residents with memory care needs such as Alzheimer's disease and dementia by implementing specialized care strategies.
Operate Electronic Health Record (EHR) systems to document patient care activities comprehensively and securely.
Perform vital signs assessments--such as blood pressure, pulse, temperature, and respiration--and report findings promptly.
Assist with medical procedures including catheter care, wound management, Hoyer lift transfers, and hospice support when needed.
Provide compassionate companionship and emotional support to residents, working with people with developmental disabilities or complex health conditions.
Maintain a clean environment by assisting with laundry, cleaning resident areas, and meal preparation to promote hygiene and comfort.
Support safe patient handling using equipment like Hoyer lifts and ensure adherence to safety protocols at all times.
Collaborate effectively with nursing staff and other healthcare professionals to deliver comprehensive patient care in assisted living or long-term care settings.
Ensure all medical records are accurately maintained in compliance with healthcare regulations such as HIPAA.
Proven experience as a Health Care Aide or similar caregiving role in assisted living, nursing homes, or home health environments.
Knowledge of medical terminology and familiarity with EMR/EHR systems for documentation purposes.
Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice care are a plus.
Experience working with seniors, individuals with disabilities, dementia or Alzheimer's care is highly valued.
Ability to assist with ADLs efficiently while maintaining a positive attitude and respectful approach.
Strong observational skills for patient monitoring and reporting changes accurately.
Physical stamina to perform lifting tasks and assist residents with mobility needs safely.
Excellent communication skills to collaborate effectively within a multidisciplinary team.
Knowledge of HIPAA regulations ensuring confidentiality of patient information.
