GENERAL DESCRIPTION:



The SHS Healthcare Aide (HCA) works with the multidisciplinary team to support clients and families with daily living activities and basic health services. This includes care for those with chronic or complex conditions, disabilities, or palliative needs, provided at home, in care settings, or in the community. The care provided is based on client needs, care setting, and the HCA's competencies.

DUTIES AND RESPONSIBILITIES:



Providing person- and family-centered care that supports the unique needs and abilities of the client/family to maintain health, safety, independence, and comfort, as outlined in the individualized care plan developed by the HCC Manager, RN, or LPN. Personal care and support for daily living activities include, but are not limited to Assisting with bathing, grooming, toileting, skin care, dressing, eating, hydration etc. Assisting with light housework, meal preparation, and social contacts. Providing regular foot and nail care to non-diabetic clients and the elderly. Ensuring safe use of home care equipment and mobility aides, as required. Promoting, guiding, teaching, and supporting the clients/family's ability to participate in self-care and to: Adhere to medical (i.e. medication management) and nursing (i.e. wound care etc.) regimes. Adhere to home exercise and strength-building programs. Participate actively in their care and be as self-reliant as possible. Monitoring and reporting client information on time on General safety in and near the home (i.e., adequate lighting and heating, even floors, safe access, etc.). Changes in the client's medical, physical, emotional, or social behaviors. Risk of neglect or abuse. Performing clerical tasks such as booking visits, documentation, follow-ups, and reporting, while ensuring accurate, confidential, and up-to-date client records. Environmental and Workplace Safety: Promoting a safe and healthy environment in the homes, the workplace, and the community. Acts as a resource for any SHS programs that require Health Care Aide expertise. Participates as a member of the Stoney Health Services team in: Quality Improvement, Risk Management, Occupational Health and Safety (OHS), Accreditation activities, and other new development opportunities. Community outreach, educational, and other health promotion/injury prevention activities etc. Team/staff meetings, training, and development opportunities. Operational development and strategic planning sessions. Acts as an ambassador for SHS, for client and family-centered care, and as an advocate for client autonomy. Carries out other associated duties as required, including SHS Home Support Worker roles and responsibilities, policy reviews, leading groups, etc.

EDUCATION, SKILLS, AND QUALIFICATIONS:



Completion of an approved training Program for Personal Care Aides. Understand and perform competencies as defined by Alberta Health Care Aide Competency Profile (2018). A minimum of six (6) months of experience in HCC or a related field. Experience with or interest in indigenous culture and an understanding of cultural diversity issues. Experience with or interest in working in community and home care settings. Ability to function independently and effectively with minimal supervision in a challenging environment. Demonstrates initiative, problem-solving, time management, and organizational skills. Good (oral and written) communication and interpersonal skills. Computer and mobile communication skills relevant to the position. Maintains an effective reporting system (daily & monthly). A graduate of a recognized Health Care Aide Program. Basic Life Support Provider Certification (BLS - Provider). Current First Aid Certification. Current Transportation of Dangerous Goods Certification, an asset. Current WHMIS Certification, an asset. Current enrollment on the Alberta HCA Directory

EMPLOYMENT CONDITIONS:



Clear Criminal record check and vulnerable sector Class 5 Advanced Driver's License and Clean Driving Record Willing and able to work flexible hours, including evenings, weekends, and holidays.
