Health Care Aide

Responsibilities
Observe and monitor clients regularly, noting any changes in condition and reporting concerns promptly.
Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
Administer medications and perform basic medical tasks--such as taking vital signs, catheter care, and wound care--under appropriate supervision.
Provide compassionate, specialized support to clients with dementia or Alzheimer's disease, using effective memory-care techniques.
Document client information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, in full compliance with HIPAA regulations.
Assist with meal preparation, feeding, cleaning, laundry, and maintaining a clean, safe, and organized environment.
Safely operate mobility and transfer equipment such as Hoyer lifts.
Travel to multiple clients in the community during PM shifts, ensuring punctual and reliable service.
Requirements
Prior experience in long-term care settings such as nursing homes or assisted living facilities is highly preferred.
Knowledge of basic medical terminology and familiarity with medical documentation practices.
Current CPR (Cardiopulmonary Resuscitation) certification is required; additional certifications such as medication administration or hospice training are considered an asset.
Experience supporting individuals with developmental disabilities, dementia, or Alzheimer's disease is an advantage.
Ability to work effectively with diverse populations, including seniors, individuals with disabilities, and those receiving home or community-based care.
Strong communication skills and the ability to work both independently and collaboratively.
Physical stamina to perform caregiving duties involving lifting, transferring, bending, and standing for extended periods.
Reliable vehicle and valid driver's license required
