The Toronto Community Hep C Program (TCHCP) is a community-based, collaborative model of Hepatitis C (HCV) treatment, support and care delivery focused on improving access to treatment for people who use drugs and/or alcohol. The HCV Community Support Worker will work within the TCHCP providing support to program members as well as supporting and assisting with the program activities. This position will work together as part of a multidisciplinary health and social service team to improve existing services and projects and in the development of new initiatives. This position is expected to work onsite.


Responsibilities:



Distribution of harm reduction materials and harm reduction education Community outreach and HCV education Co-facilitation of groups, supervision of drop-ins Public speaking, public education and advocacy One-on-one support to program members including appointment accompaniment Referrals to appropriate resources Assist with preparation for groups, special projects and community events Support members through testing results, living with HCV and/or assessing and sustaining treatment for HCV Assist with research, surveys and program evaluation Maintaining accurate records and documentation of services Attend meetings and trainings Maintain confidentiality and appropriate boundaries Act as a role model for program clients All other duties as required

Qualifications:



Graduate from Peer training program or equivalent Lived Experience Excellent knowledge and practice of Harm Reduction including substance use resources and supports Knowledge of HCV/HIV Basic knowledge of computers, email and internet Experience in navigating systems and advocating for illicit drug users rights/needs including the criminal justice system, medical and hospital systems, mental health systems, traditional drug treatment systems, etc. Demonstrated sensitivity to others Respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and positively with others Ability to function as a member of a team with good interpersonal skills Good verbal and written communication skills including ability to actively listen Open to feedback Good organization andcoordination skills Confident in establishing connections and maintaining strong boundaries Ability to prevent and de-escalate conflicts Ability to be punctual and work consistent hours Police clearance documentation will be required for employment Proof of double COVID-19 vaccination status will be required for employment
