As a member of the Phoenix Centre Mental Health and Substance Use Team, the Social Worker, at Phoenix Society participates in the recovery and capacity building of residents to maximize their social and economic potential and support their goals for successful independent living in the community.
Responsibilities:
Complete bio-psycho-social assessments with clients
Facilitate case planning meetings as needed with youth, Counsellors, Case Managers and any other supportive parties
Liaise with other community service providers and program services staff for the purpose of case management
Ensure consultation and collaboration among teams on behalf of the clients
Coordinate community case conferences and internal case review meetings as necessary
Attend inter-agency and program related meetings as required
Collaborate and advocate with front line staff to ensure ethical and clinically sound case plans
Provide workshops depending on areas of interest, need and expertise
Oversee and/or supervise the "learning practicum" of students in the department as assigned
Liaise between programs and health teams from all over the province
Represent Phoenix Society in the community and work at building relationships community partners
Liaise with in-house service providers and community service providers
Attend community meetings and work on joint service projects as required
Referral coordination
Other duties as required
Qualifications:
Bachelor's degree (e.g., 2 to 3 years) in social sciences, social work, etc.
Minimum one (1) year of experience working with marginalized populations, substance use services, mental health, corrections, etc., or equivalent
Must be registered as a Social Worker (British Columbia) with the BCCSW (or the province you currently reside in but be looking to register in BC).
Experience with direct service to youth/adult population including crisis intervention and supervision experience is an asset
Experience working with people with mental health and substance use issues
Satisfactory completion of a criminal records check with vulnerable sector screening is required
Satisfactory completion of Tuberculosis Screening form is required
Satisfactory completion of Employee Immunization Record form is required including COVID19 immunization records
First aid certification with CPR is required
Valid class 5 driver's license is required
Skills & Knowledge:
Knowledge of Windows, Microsoft Word, Excel, and database management systems
High level of integrity and dependability with a strong sense of urgency and results-orientation
Strong problem solving, analytical, and creative skills with the ability to exercise sound judgement and make decisions based on accurate and timely analyses
Strong interpersonal skills and ability to communicate effectively in writing and verbally in English

Demonstrated therapeutic skills in the areas of case management, individual and group therapy with adults with substance use and/or concurrent disorders
Ability to form effective collaborative relationships with clients utilizing strong relationship-building and de-escalation skills
Ability to work in a trauma-informed framework
Demonstrated commitment to the social sector with a passion for the organization's mission is essential

Salary for this role is between
$36.59 - $45.66
per hour.
