Since 1995, Hospice Vaughan, a not-for-profit organization, has been helping people with life-limiting illnesses live fully in comfort and with dignity until they die, while providing support for families, friends, and caregivers. We provide a 12-bed residential hospice as well as a range of community services, including service navigation and case management, grief and family support services, day programs, and complementary therapies and wellness programs.

This role will appeal to a dynamic and experienced regulated health care professional who values hospice palliative care and is eager to support a growing organization and an energetic team. Reporting to the Manager of Residence, the Grief and Supportive Care Counsellor will provide support to individuals living with and dying from life-limiting illnesses and their family members in the residence.
Primary Duties and Responsibilities
Work as part of an interdisciplinary team and strong core of volunteers to provide care to individuals/caregivers at the end of life with compassion, empathy, and understanding
Provide quality support to residents and their caregivers
Work in close collaboration with other members of the clinical team
Facilitate family meetings and resident interactions in a way that meets resident and caregiver needs
Lead debrief support to staff
Offer support based on sound knowledge of hospice palliative care and grief processes
Provide grief education and support for any resident/family member who requests help
Participate in group memorial service for staff, the bereaved, and the community
Support, instruct, and educate the resident and family/caregiver
Support the resident and family's unique spiritual and cultural beliefs
Collaborate with other agencies and professionals for the benefit of the resident
Provide mentorship and supervision to students as appropriate
Support grief and supportive care programming for community clients as needed
Position Requirements
Licensed/certified as a regulated health professional in Ontario
University degree in Psychology, Counselling, Social Work, or equivalent with expertise in hospice palliative care; a master's degree is an asset
Previous grief counselling experience
Demonstrated understanding of hospice palliative care
Experience working with an interprofessional team
Experience working with volunteers and diverse populations
Strong computer skills (e.g., Word, Excel, Outlook, Teams, Zoom, etc.)
Knowledge of Hospice Palliative Care Ontario standards
Completed palliative education
Required to work fully on site
Willingness to perform any other duties to ensure that Hospice Vaughan operates in an effective and professional manner
