Current Rate of Pay: Min $44.20- Max $55.24
Current Shifts: Primarily Days (Temporary Full-time for approximately 6 months)
Position Summary:
The GGH@Home Patient Navigator is responsible for collaborating with patients and their families to develop high-quality, timely, and cost-effective individual service plans based on patient needs. Utilizing a multidisciplinary approach, the goal is to achieve optimal health outcomes.
The primary purpose of this role is to assist patients in reaching their highest level of functioning and independence, in alignment with their values, priorities, capacities, and preferences for care. The GGH@Home Patient Navigator will determine eligibility for services in accordance with relevant policies and procedures and care pathways.
The GGH@Home Patient Navigator will provide comprehensive clinical services to patients and their families. This role includes crisis intervention, emotional support, consultation, and counseling, completing referrals, and connecting patients to appropriate community services. The GGH@Home Patient Navigator will lead efforts to educate and counsel patients and families while facilitating referrals to support services, all aimed at addressing patients' goals.
The GGH@Home Patient Navigator will liaise between the hospital care team including physicians, primary care and community support services partners including Ontario Health at Home.

Qualifications, Experience, Skills and Abilities:
  • Valid registration with a Professional Healthcare Regulating Body
  • Completed Baccalaureate Degree; Masters preferred
  • Minimum 3-5 years acute care hospital experience, preferably in a medical/surgical area.
  • Project Management experience an asset
  • Knowledge and experience with Long Term Care strategies, Alternate Level of Care (ALC) designation and discharge processes
  • Must be able to determine capacity in accordance with the Health Care Consent Act (1996)
  • Employee must demonstrate basic computer skills and literacy
  • Ability to work with a diverse stakeholder group with demonstrated facilitating and coaching clinical areas on change initiatives
  • Self-directed, courageous, and highly motivated with excellent interpersonal and communication skill
  • Experience managing competing demands in a fast-paced, multi-disciplinary health care
  • Evidence of effective communication and conflict resolution skills with the ability to develop positive working relationships with all stakeholders promoting collective problem solving approaches;
  • Excellent interpersonal skills
  • Demonstrated ability to work independently and in a self-directed manner as well as collaboratively within a team in a fast-paced and ever-changing environment
  • Demonstrated commitment to excellent customer service when interacting with patients, families, colleagues and physicians
