Reporting to the Director of Home-Based Care the

Program Manager - Geriatrics Program Leader

is responsible for leadership, supervision, administration, management and development of the provincial geriatric program through operational and strategic planning to ensure optimal older adult (geriatric) care. The incumbent is responsible for ensuring the geriatric program services/initiatives align with the broader continuum of care and within a provincial context based on evidence and system performance. The Provincial Geriatric Program provides individualized comprehensive geriatric assessment to older adults with complex diagnostic care and support needs. The program serves the province of PEI providing clinical patient care (consultation and assessment) and education responding to referrals from primary practitioners requesting specialized geriatric assessment of patients

Duties:



Establishes geriatric program operational direction in alignment with divisional and organizational strategy, including development of program operational priorities, goals, objectives, and tactical plans Assesses and monitors program within broad system and/ or within specific program area to support optimal geriatric program development, implementation and evaluation. Establishes and maintains policy development framework for geriatric program, including processes for development, monitoring, execution and evaluation Provides regular written and oral advice/reports to the Director, Home Based Care on program activities and other organizational forums (including committees) as required Liaises with Director Home based Care and the Provincial Medical Director Seniors Care Health PEI to determine strategic programming and ongoing development of service provision Leads and directs program staff on a regular basis to discuss and advise on operational challenges and opportunities for development and enhancement Coordinates and integrates clinical services with program operationalization. Oversees implementation and evaluation of quality improvement, risk management and utilization processes Provides change management leadership and support at program / staff level Responsible for administrative management of medical secretaries, NP's, OT/PT Accountable for general administrative requirements of physician's geriatricians Responsible for clinical resources including recruitment, performance management, staff development, confirmation of professional certification/ license Interpreting policies, procedures, goals and objectives with employees Chairs and arranges regular staff meetings, conduct working groups as required This position reviews and analyzes all referrals through a central intake process and promotes client centered care by streamlining all referrals. Central intake of the referrals ensures timely and equitable access to care. Collaboration with referral sources, clients and caregivers to gather information. Additional medical, pharmacological and collateral information is obtained through direct contact with the referring Physician/Nurse Practitioner if information is not clear or additional information required. Multiple electronic client information systems are used to gather information. Once the referral is deemed appropriate based on clinical judgement and critical thinking the Manager would use those skills to determine the appropriate service in a timely manner to receive an appointment to be seen by either a Geriatrician or Nurse Practitioner or be waitlist for assessment in the next year. Collaboration with referring source if client's condition has changed since initial referral and requires to be seen urgently. Information is gathered and it is determined if the clients needs to have an adjustment in appointment. Acts as an enabler for coordinated access to Geriatric care. Ensures prioritization and accommodation of those with highest levels of need first based on clinical judgement Effectively and actively manages the waitlist. Ensure development of effective teams maintaining a healthy and safe work environment, for staff and patient Reviews population needs and best evidence practice ensuring congruence of programs and services with provincial priorities, goals, objectives and outcome indictors Liaise regularly with geriatric program medical director to maintain open communication on related initiatives and issues Manage complaints and incidents within the geriatric program Assess, investigate and report on critical issues and assume responsibility for assisting in effective problem solving and resolution as appropriate In conjunction with the Provincial Medical Director Seniors Care Health PEI and Home-Based Care Program leadership, proactively identify and pursue collaborative opportunities with system partners, including non-governmental organizations, to advance strategic agenda Maintains budget responsibility for Provincial Geriatric Program Ensures adequate staff to provide program services. Develops specifications for request for staffing proposals

Minimum Qualifications:



Baccalaureate in Nursing with extensive experience working with the Geriatric population. Master's Degree would be an asset. Current registration with the College of Registered Nursed and Midwives of PEI CNA certification in Gerontology required or willingness to obtain within two years of hiring. Expertise in and comprehensive well-developed knowledge of Geriatric care and syndromes. Demonstrated commitment to ongoing professional development as a leader through pursuit of a recognized Canadian health professional designation or be willing to participate in the Canadian College of Health Leaders (CCHL) Certified Health Executive (CHE) Program or equivalent Thorough fluency in the LEADS framework capabilities with an ability to lead an interprofessional team independently and with minimal supervision within a defined scope of responsibility Demonstrated leadership/management experience in health care field including budget management, risk management, strategic planning, policy and program development and evaluation processes. Management level experience in human resource management within a multi union environment, change management and conflict resolution. Extensive management experience and proven ability to work effectively with multiple health disciplines and staff in a collaborative team environment. Working knowledge of PEI health system health sector, collective agreements, performance management principles, employer/labor relations. Experience communicating with managers, physicians, health care professionals, other governmental and non-governmental organizations, media, and members of the general public Strong interpersonal and collaborative skills and the ability to plan and think strategically, foster a shared vision, accept accountability and integrate evidence into decision-making Ability to develop progressive and effective partners across a multitude of sectors and engage multiple stakeholders to achieve a common goal. Excellent organizational, communication, facilitation and presentation skills Self-awareness-. honesty, integrity, optimism, confidence and resiliency Highly motivated, conscientious and able to problem solve under stressful conditions and deadlines

Other Qualifications:



Computer knowledge including spreadsheet applications, word processing and presentation software. Experience with Electronic Health Records and Incident reporting Software A valid driver's license / access to reliable transportation. Good previous work attendance

Salary Range:

$45.76 - $56.29 per hour (Level 20N)

Bi-Weekly Hours:

75 Hours Bi-Weekly / Mon - Fri
