Duties

Conduct assessments to identify clients' needs and develop personalized service plans Provide counseling, support, and resource referrals to clients facing various challenges Monitor client progress and adjust service plans as necessary Collaborate with community organizations, agencies, and service providers to coordinate care Maintain accurate and confidential case records in compliance with organizational policies Advocate on behalf of clients to ensure they receive appropriate services and support Supervise or mentor junior staff or interns as needed, ensuring quality service delivery Facilitate group sessions or workshops aimed at skill development and resource awareness

Requirements

Proven experience in case management or social services preferred Supervising experience is highly desirable to oversee team activities effectively Strong interpersonal and communication skills with the ability to build trust with clients from diverse backgrounds Excellent organizational skills with attention to detail in documentation and reporting Ability to handle sensitive situations with empathy, professionalism, and discretion Knowledge of community resources, social services systems, and relevant policies Bachelor's degree in social work, psychology, or related field is preferred but not mandatory depending on experience

Job Type: Full-time

Pay: $44,244.93-$60,000.00 per year

Work Location: In person
