The Program Coordinator supervises and oversees the services and day-to-day operations of the Assisted Living Program. The Coordinator is actively involved in providing direct service to clients as well as providing support and supervision to staff. The Coordinator works with the staff, Manager, Registered Nurse, Fraser Health teams, and other partners in building a skilled, cohesive, collaborative approach to service. The Coordinator also works with the Manager to ensure that program delivery is consistent with the policies, procedures, and Mission Statement of Progressive Housing Society.

QUALIFICATIONS
:
Bachelor's Degree in Social Work, Psychology, or other related field preferred; may consider equivalent combination of education and experience
5 years related experience in mental health
Minimum 2 years supervisory experience in the Not For Profit or health sector
Valid Class 5 Driver's License

KEY DUTIES AND RESPONSIBILITIES:
Supervises and oversees the services and day to day operations of the Assisted Living program, including: establishing resident & staff schedules, monitoring health & safety of staff and clients (including inspections and hazard reporting),
cleaning, providing a supportive environment to clients, ensuring basic needs are met, and reporting critical incidences.
Develops, implements, and evaluates goals, objective, and policies as required and directed by the Manager
In addition to performing the duties and responsibilities of Assisted Living Support Worker, also provides support and supervision to staff.
Participates in person-centred case planning in collaboration with clients, staff, manager, clinical partners, as well as other community resources and family as appropriate. Coordinates services with professional groups such as nurses, doctors, case managers, etc. Ensures that plans are updated at regular intervals.
Supports the mental health wellbeing, recovery, and psychosocial support of clients in the program, including crisis intervention. Oversees staff in actively supporting goals and skill development.
Responds to and reports on client changes, emerging issues, and crisis situations. Develops plans (including Safety Plans) as needed, and oversees implementation.
Supervises staff by performing duties such as assigning work, providing feedback and evaluation, determining training requirements, orienting new staff, and maintaining timekeeping and attendance records. Resolves staffing problems, including calling in staff to ensure appropriate staffing levels.
Consults with community groups and health care professionals to identify trends, needs, and priority health issues.
Oversees ongoing implementation of medication administration progam.
Ensures proper record-keeping and documentation, including reporting and program evaluation.
Works with the Manager and staff in building a skilled, cohesive, collaborative team.
Ensures the maintenance and cleanliness of residence(s) including: cleaning, clean-out of belongings, light maintenance of home and yard, overseeing maintenance work and contractors, scheduling servicing and repairs, etc.
Facilitates intakes and discharges under Manager's direction, ensuring that policies, orientation, and documentation are completed accurately.
Works with the manager to ensure that program delivery is consistent with the policies, procedures, and Mission Statement of Progressive Housing Society.
Other related duties, as required.
