McMan is looking for a Full Time Youth Mentor for the Transition to Independence Program (TIP) in Lethbridge.

About the position:



Under the supervision of the Program Supervisor, the Youth Mentor is responsible for ensuring that youth assigned to your caseload will receive the appropriate supports to obtain suitable housing and maintain their tenancy through the use of Intensive Case Management. Your work will be guided by Housing First, Family First and Harm Reduction philosophies, as well as being service participant-focused and strengths based.

Here's how you will contribute as a Youth Mentor within the Transition to Independence Program

:

Manage a caseload as assigned by the Program Supervisor including all required related administrative functions including case notes, documentation, reassessments, forms, applications, etc. Identify and connect youth to appropriate wrap around supports. Provide ongoing assessment, case management, care coordination and monitoring of youth's needs; monitor and evaluate youth's changing needs and coordinate individualized services. When appropriate, assist youth to identify and access alternative courses of action when community-based services are not appropriate to the needs of the individual.

Here's what you'll need to get an interview:



Degree/Diploma in Human Services, plus experience working with youth, case management, intervention, wrap around supports and advocacy. A combination of related education and experience may be considered. Experience in building relationships with community resources. Knowledge of existing community resources and understanding of issues related to youth homelessness, including poverty, mental health, and addictions. Strong documentation and assessment skills. Current Standard First Aid CPR Level C Certificate or CSA Standard Z1210-17 Intermediate First Aid Certificate. Clear Child Intervention Record Check, dated within the past six months. Clear Police Information Check with vulnerable sector, dated within past six months. Valid Class 5 Driver's License, 3-year Driver's Abstract, and use of reliable vehicle with $2M Third Party Liability insurance.
