The Wraparound Supports Case Worker is responsible for providing support, coordination and delivery of all aspects of Wraparound Supports.

Wraparound supports program connects youths with service agencies and supports that enable a means of housing stability. These supports will include but are not limited to life skills, advocacy, housing stability support warm transfer referrals, mental health support coordination, harm reduction, stability planning that will contribute to successfully getting housed and/or staying housed.

KEY RESPONSIBILITIES:

Program

  • Develop comprehensive community contacts and resources
  • Develop service agreements with partner agencies
  • Perform client intake and needs/skills assessment for Transition, Drop In and recently housed clients
  • Utilize approved case management tools
  • Identify individual strengths and assets to sustained tenancy
  • Coordinate, refer and deliver all needed wraparound supports.
  • Provide warm transfers to requested partner agency staff
  • Participate in multi-agency information sharing case management reviews, as required, follow through on any action plans and monitoring same on a regular basis
  • Create, implement and monitor a plan of support
  • Perform performance feedback to Wraparound Supports client
  • Provide advocacy to Landlords
  • Collect statistical information related to the clients in program
  • Perform any other related duties as requested by Program Services Director
Administrative Responsibilities
  • Prepare case notes (intermittently throughout each working day) and share them via electronic means to relevant individuals involved in the client's circle of care while maintaining client's confidentiality
  • Ensure that the privacy and confidentiality of clients is maintained within established standards, Policy and Procedure
  • Presents case reports and progress of clients at regularly scheduled team meetings
  • Be involved in ongoing professional development and to attend Staff / Development meetings.
  • Work as a member of the team to ensure that the program operates efficiently and effectively (and to ensure that other staff's responsibilities are covered when they are away).
  • Participate in and represent the organization at external committees & forums.
  • Participate in internal committees and/or activities relating to the organization (e.g. joint staff, annual general meeting, other social recreation events, etc.).
  • Perform other related duties as requested from time to time by the Program Director.
Service Responsibilities
  • The incumbent will promote positive and supportive attitudes toward youths who are experiencing homelessness
  • Promote programs and activities that empower youths to work towards their goals
  • Act as an advocate and be a positive role model for the youths in the community
  • Develop effective working relationships and partnerships with community members, representatives of other departments, agencies and co-workers
  • Represent the Salvation Army and the youths we serve at community meetings
Organizational Responsibilities
  • The incumbent will adhere to standards that are consistent with the values & philosophy of The Salvation Army and the Residential Services Department and the Regional Municipality of York.
  • Participate fully and constructively in staff meetings and in service training
  • Engage in formal evaluation sessions with Program Services Director at 3 months during first year of employment and annually thereafter
  • Ensure all employees read and follow the Policy & Procedures Manual and the Employees Handbook
  • Maintain confidentiality in all matters pertaining to clientele and coworkers of Sutton Youth Services.
  • Become familiar with the provision of the position description & operations manual, which will be provided for each job position
  • As required and is applicable, make suggestions that will improve efficiency, working conditions or procedures to Executive director; and represent the Organization in a professional manner
Perform other duties as required.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:
  • A degree or diploma in Social Science and/or equivalent education and experience
  • Valid Ontario Class "G" Driver's License and a current copy of a Driver's abstract for review and verification that is satisfactory to The Salvation Army, in its sole discretion, is required
  • Alternative combinations of education and experience may be considered
EXPERIENCE AND KNOWLEDGE:
  • Experience in coordinating wraparound supports
  • Previous experience with homeless Youths (one to three years)
  • Must have thorough knowledge and understanding of Youths issues, Mental Health and Addictions issues, Health and Safety issues, Microsoft Software
  • Knowledge of HRDC guidelines
  • Good knowledge of community resources
  • Thorough knowledge and a commitment to anti-racism, anti-oppression and equity
SKILLS AND CAPABILITIES:
  • Ability to work independently as well as part of a team
  • Problem solving skills essential
  • Must have good interpersonal/time management skills
  • The responsibilities associated with this position demand a high degree of confidentiality
  • Excellent oral and written communication skills (Processing/Desktop, Publishing, Spreadsheet/Graphing, Database computer skills essential)
  • Ability to undergo applicable screening and background checks successfully that are satisfactory to The Salvation Army, in its sole discretion (i.e., The Salvation Army Abuse Registry) and provide an original copy of a Background Check with vulnerable sector screening
  • Ability and willingness to develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities
