Job Description

Fieldgate Retirement Living is part of Fieldgate Properties, a full-service real estate investment, property, and asset management company. We focus on the acquisition, development, and management of retirement communities. From new builds to redevelopment and expansion, Fieldgate has a vast portfolio of projects either currently under development or completed throughout the GTA.

We are looking to hire a full time Unregulated Care Provider (UCP) to work out of our Oakville location.

The UCP is required to provide, and assist in the planning of, personal care to each resident under the direction and supervision of the Nursing Supervisor/designate.

FUNCTIONS:

1. Performs personal care and selected nursing measures to promote the comfort and safety of the residents under the supervision of a Registered Healthcare Professional such as a Registered Nurse (RN) or a Registered Practical Nurse (RPN)

2. Participates in the overall activation process developed by the facility to promote the well-being of each resident.

3. Attends the care conferences as requested by the Nursing Supervisor/designate to assist in the formulation of a comprehensive plan of care for each resident.

4. Observes and promptly reports changes in a resident's physical and emotional condition.

5. Maintains accurate documentation, according to established policies, reflecting each resident's condition, care, and treatments.

6. Deals tactfully and courteously with residents, residents' families, and visitors.

7. Directs all inquiries regarding residents' care and conditions to the Charge Nurse.

8. Is aware of the Resident Rights and Responsibilities and endeavours to respect and promote it.

9. Assists in maintaining a tidy and clean environment.

10. Attends staff meetings as required - to know changes in the facility that have taken place and update knowledge in better caring for the residents.

11. Reports all accidents and incidents promptly and accurately.

12. Shares responsibility for the care of supplies and equipment as well as the personal belongings of the residents.

13. Attends in-service programs to better care for the residents.

14. Performs other related duties as requested by the Nursing Supervisor/designate.

15. Knowledge of home care services, both private pay and government funded

QUALIFICATIONS:

1. UCP Certification

2. Genuine interest in and concern for seniors.

3. Proficient in oral and written communication and computer documentation

4. Experience in long-term care or retirement home desirable.

5. Maturity and good physical and mental health.

REPORTS TO:

The UCP reports directly to the Nursing Supervisor/designate.

WORKING CONDITION:

Must be able to stand for long periods of time and walk long distances during work period.

Lifting alone, maximum 40 lbs. (5-25 lbs. usual) as an essential job function.

Lifting with assistance, maximum 160 lbs. (100-200 lbs. usual) as an essential job function.

Pushing/pulling, using wheeled equipment such as carts and trolleys, maximum 100 lbs. (20-50 lbs. usual) as an essential job function.

Stretching/reaching on a regular frequency throughout the shift as an essential job function.

Walking, with possible some upstairs walking, up to 3 hours maximum, (1-2 hours usual) as an essential job function.

Standing, for lengths of time, up to 3 hours maximum (1-2 hours usual) as an essential job function.

Bending/stooping on a regular frequency throughout the shift, as an essential job function.

Crouching/kneeling on an irregular frequency throughout the shift, as a non-essential job function.

Carries/transports objects of various sizes, up to a maximum of 25 lbs. (5-15 lbs. usual) as an essential job function.

Must be able to perform turning, stooping, bending, stretching, and lifting to assist residents in care requirements and other related duties.

Must be able to move and lift equipment and supplies of various sizes and weights.

Must have visual and hearing acuity to recognize the need of assistance required by residents.

RESPONSIBILITIES TO HEALTH AND SAFETY:

1. Is aware of employees' responsibilities and follows all health and safety policies and procedures as set out in the employee safety program handbook and departmental manuals.

2. Works safely to reduce the risk of injury to self, co-workers, and residents.

3. Is alert to and promptly reports all actual or potentially hazardous situations to immediate supervisor. Does not operate or use faulty equipment.

4. Wears personal protective equipment (or clothing) as required by task, M.S.D.S. or facility policy.

5. Promptly reports personal injury to supervisor and seeks first aid as needed.

6. Participates in fire safety demonstrations and fire drills and knows the facility fire and disaster plan.
