```Duties```

- Dispense prescription medications to patients and provide appropriate medication counseling
- Collaborate with healthcare professionals working in the same facility
- Review patient medication profiles for potential drug interactions or contraindications
- Monitor patient response to medication therapy and adjust treatment plans as necessary
- Provide accurate and timely medication orders for inpatient and outpatient settings
- Maintain proper storage and handling of medications in accordance with pharmacy regulations
- Participate in medication reconciliation processes to ensure continuity of care
- Educate patients on proper medication use, potential side effects, and adherence strategies

```Experience```

- Licensed with CPHM
- Excellent communication and interpersonal skills
