ROLE PURPOSE



This position will provide outreach support to client(s) referred to Connective by Community Living British Columbia to enable their adjustment to, and success with life in the community.



STATUS AND CLASSIFICATION



Unionized. This position is open to applicants of all genders.



HOURS (40 Hours Per Week)



Monday to Friday, 0800 - 1600.

REPORTING AND WORKING RELATIONSHIPS



Reports and is accountable to the Program Manager or their designate Participates as a member of Connective team to ensure quality and continuity of services to clients Liaises with staff and volunteers of community and government agencies as required Liaises with Connective staff as necessary Accepts clinical direction from the care team Participates as a member of Connective staff team

KEY ACCOUNTABILITIES



Receives referrals from CLBC Works with the client, researches background material, and seeks the input of other professionals and family members, to provide the assessment that will ascertain needs and postulate intervention possibilities Supports individuals in their housing/residential settings Works with the client to consider issues and creates a mutually agreed intervention and support plan that has measurable short and long-term goals Ensures issues of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure lifestyle and access, employment and education are addressed in this plan Ensures clients' ongoing legal, mental, and physical health issues are addressed in this plan When and as needed provides a wide range of interventions to enable the client to address and accomplish the established goals Interacts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client's interest and supportive of the long-term goals On the clients' behalf, is a resource to other professionals, including but not limited to the courts, probation services, and community and institutional psychiatric services Maintains extensive records and case notes in accordance with CLBC policies and standards Completes monthly progress reports Participates in staff and tenant/staff meetings as instructed by the Program Manager Other duties as subsequently negotiated

WORKING CONDITIONS



Functions independently, while remaining a crucial member of the care team Adheres to Connective policies and procedures Is available to work flexible hours as needed, which may include some evenings/weekends, as determined by program/client needs and schedules Is a member of CUPE Local 1936-11 Receives benefits and works within the working conditions as outlined in the collective agreement and personnel manual

EXPERIENCE AND SKILLS



Graduate with a Diploma or Certificate in Social Services and one-year of relevant experience is preferred Experience with clients with acquired brain injuries, a history of conflict with the law, developmental disabilities and/or spectrum disorders, interaction with social agencies and experiencing psychiatric/personality disorders A demonstrated and clear ability to respond well in crisis situations Understanding of providing service to people with different learning Abilities Extensive knowledge of community services as they relate to persons with psychiatric involvement Experience in assessment, case/care plan development and implementation Demonstrated ability to deal with potential violence Demonstrated ability to develop programs tailored to the particular diverse needs of the individual Professional or lived experience working with vulnerable populations experiencing multiple barriers Excellent written and verbal communication skills in English

REQUIREMENTS



Ability to successfully pass a reference check Ability to successfully complete a Vulnerable Persons Criminal Records Check Current Emergency First Aid Certificate: Certificates accepted in B.C. - WorkSafeBC Valid Class 5 driver's license Personal transportation that is in good repair, insured for business up to $1,000,000 for liability, and which can be used to transport clients Proof of vaccination for communicable diseases may be required as mandated by Government, Health Authority, and/or funder requirements

COMPENSATION



As Connective is a unionized environment, the starting wage is $27.54 per hour. Plus, benefits enrollment as per the Collective Agreement.
