The position may involve travel to all branches and hospital sites throughout the Central East region.Please note that the initial assignment for this Float Care Coordinator position is in the Community program with a Team assignment and a regular hours schedule (initial assignment subject to change).This Care Coordinator will be responsible for assessing patient eligibility for services/long term care placement, assessing patient care needs, and planning and authorizing service delivery to patients that best meet the needs of the patient within the mission, vision and values of Home and Community Care Support Services Central East. This Care Coordinator will work independently and within a team to maximize quality outcomes for patients, as well as the organization.What must you have?Membership, in good standing, with the applicable regulatory body:
o College of Nurses of Ontario
o College of Physiotherapists of Ontario
o College of Occupational Therapists of Ontario
o College of Audiologists and Speech Language Pathologists of Ontario
o Ontario College of Social Workers and Social Service Workers
o College of Dietitians of Ontario2+ years of recent experience in community health or a related fieldKnowledge of the health care delivery system and community resourcesExcellent interpersonal, communication, assessment, problem-solving and decision-making skillsEffective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environmentEstablished ability to accurately complete required documentation, reports and formsA valid driver\'s licence and access to a reliable vehicleProficient in a Windows environmentWe have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.Ability to complete 3-4 home visits daily, which may involve substantial driving or walking depending on the location of the visit, and includes patient assessments, forms completion (electronically and manually), and communicating with patients and families to ensure their understanding.Other duties as assigned.What would give you the edge?Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatricsCase management experience or recent related community experienceAbility to speak French or another second languageHours of WorkAll hours of operation (Initially Monday-Friday 0830-1630; 70 hours per bi-weekly pay period) subject to change as per the Collective Agreement.Position Location and TravelThe position may involve travel throughout the Central East geographic region.
