The Alzheimer Society of Durham Region (ASDR) is a community support service organization that provides programs and services to people living with dementia and their care partners in Durham Region. We work closely with other service providers to ensure quality care and support for our clients to achieve our mission to improve the quality of life of people living with Alzheimer's disease or a related dementia and their care partners.

Under the direction of the Client Services Manager, the First Link Care Navigator will be responsible for, facilitating early intervention, dementia education, and fostering connections and linkages for the individual seeking service and support from Alzheimer Society Durham Region. The First Link Care Navigator will also assist in the facilitation of support and education groups.
The role of First Link Care Navigator will also fulfill all expectations outlined in agency Policies and Procedures.
What you will be doing
Gather information, conduct or review relevant assessments, and meet with clients (people living with dementia and care partners) to identify current and future needs, goals and level of risk.
Establish appropriate intervention/care plans to meet bio/psycho/social needs using a person/family-centered approach guided by a citizenship model
Support clients in navigating the system to access appropriate learning opportunities, support services, care and resources as identified in their individualized plan of service
Pro-actively facilitate internal and external linkages, communication, information exchange and coordination between clients and service providers along the continuum of care
Monitor and provide proactive follow-up (in-person, phone, online) for clients and care partners to ensure ongoing collaboration across services/providers and to identify opportunities for new or emerging care needs
Provide support and education to persons living with dementia and care partners as they transition through different parts of the health, social and residential care system in both individual and group consultation format
Facilitate support and education sessions using technology, as well as traditional modes of delivery (i.e. in-person service delivery)
Maintain an advanced level of knowledge of Alzheimer's disease and other dementias, including clinical manifestations, responsive behaviors, current care practices, treatment options, placement options, available community resources, and all relevant legislation.
Collect, maintain and report required quantitative and qualitative data to support province-wide monitoring, evaluation, and reporting
Maintain confidential, accurate and current client records, including complete and thorough documentation for each client contact, in compliance with relevant privacy legislation and in accordance with professional standards and internal policies
Ensure that client consents, privacy, and confidentiality are maintained in compliance with legislation, professional standards/regulations and internal policies
Support awareness of First Link to health professionals, service providers and other relevant community stakeholders in collaboration with internal and external partners
Participate in knowledge transfer and exchange and collaborate with Alzheimer Societies across Ontario to support the delivery of best practices and ongoing quality improvement
Facilitate support groups for persons living with dementia and their care partners
Engage with clients (persons living with dementia and care partners) to develop and co-design programs that enable peer-to-peer connection and support.
Oversee programs that support health promotion and wellness for people living with dementia and their care partners, maintaining waitlists, equipment, and ensuring services are delivered in a timely manner (e.g. robotic pet adoption, iPads)
Administrative/ Organizational Responsibilities
Maintain accurate statistical data & provide reporting on a timely basis Participate in all ASDR staff training and meetings
Commit to ongoing professional development and advancement of clinical skill set Promote health and safety, equity, and inclusion of patients/clients, staff and self
In consultation with Leadership team and other department staff, assist with implementing new initiatives to support the strategic direction of ASDR
Ensure quality of service provision and contribute to continuous quality improvement initiatives
Active participation in agency related meetings and committees, providing input and involvement as a direct client care staff.
Participate in internal/external committees on an ad hoc basis, and represent the Society on community committees as assigned
Participation and involvement in key agency annual fundraising events as required Other duties as assigned from time to time

Job Qualifications
Education:
Post secondary degree/diploma in the health or social work sciences with a comprehensive knowledge and understanding of Alzheimer Disease and related dementia.
Registration with a professional college (i.e. Ontario College of Social Workers and Social Service Workers) is considered an asset
Experience:
Minimum of 2 years' experience with administration of assessments
Minimum of 3 years' experience working with persons with dementia and their care partners is preferred Related experience in a community-based health/social services role
Experience facilitating support groups is an asset
Other Knowledge, Skills, Abilities or Certifications:
First Aid/CPR-level C certified
Additional dementia-related training in U-First! GPA, PIECES, Dementiability, or Montessori Methods is an asset
Mental Health First Aid/Suicide Prevention training is an asset
Excellent verbal and written communication skills
Fluency in English, both spoken & written, is required
Fluency in French and/or a second language is preferred
Ability to work cooperatively with volunteers and staff
Excellent organizational & time management skills with the ability to prioritize and manage multiple tasks
Excellent computer/digital literacy and proficiency in Microsoft Office, video conferencing platforms, and client database software
Demonstrates initiative, flexibility and resourcefulness
Knowledge of available community resources and familiarity with Durham Region support services
The successful candidate will be required to provide a Vulnerable Persons Check (police check) conducted within the last two months.
Current Ontario driver's license and access to a vehicle and proof of $1 million insurance liability coverage
Travel Requirements
Must have insured vehicle and be able to travel throughout Durham Region
