Job Summary

The Health Care Aide plays a crucial role in providing compassionate and supportive care to patients in various settings, including assisted living facilities, nursing homes, and home health environments. This position involves assisting individuals with daily living activities, monitoring their health status, and ensuring their comfort and safety. The ideal candidate will have a strong background in patient care and a genuine desire to improve the quality of life for those they serve.

Responsibilities

Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation. Monitor and record vital signs, including temperature, pulse, respiration, and blood pressure. Provide support for individuals with developmental disabilities and those requiring Alzheimer's or dementia care. Administer medications as prescribed and ensure proper documentation in Electronic Medical Records (EMR) systems. Utilize medical terminology effectively to communicate patient needs and changes in condition. Employ specialized equipment such as Hoyer lifts for safe patient transfers. Maintain cleanliness and organization of the patient's living space, including laundry and cleaning duties. Uphold HIPAA regulations to ensure patient confidentiality at all times. Collaborate with healthcare professionals to develop and implement personalized care plans. Provide companionship and emotional support to enhance the well-being of patients.

Skills

Strong observational skills for monitoring patient conditions effectively. Experience in assisted living or long-term care environments is preferred. Proficiency in medical documentation practices and familiarity with EHR systems. Knowledge of CPR techniques and emergency response protocols. Ability to provide hospice care with sensitivity towards end-of-life issues. Experience cooking nutritious meals tailored to individual dietary needs. Competence in performing catheter care and understanding related medical procedures. Familiarity with home & community care practices is an asset. Excellent communication skills for interacting with patients, families, and healthcare teams. This role requires dedication to fostering a supportive environment for patients while ensuring their physical and emotional needs are met with dignity and respect.
