Location: Fort St James / Takla



Employment Type: Full Time - 35 hours per week

The Opportunity:



Perform assessments, implement goal plans and collect data and charting on progress Develop community programming based on demonstrated need and program objectives as outlined in funding contracts, community plans and individual goal plans. Provides services to clients supported by evidence, research and multidiscipline expertise on the wide range of health, social, and program specific issues related to health care services within First Nation communities and CSFS clinics. Collaborates with other multidisciplinary team members and physicians to facilitate an inter-professional approach to standards of practice with a focus on promoting team-based care.

What You Will Bring to The Role:



Social Services Diploma with two years extensive experience; or A combination of education and experience may be considered

, incorporated in 1952, has a population of approximately 4,700, including surrounding First Nations and rural communities. It is a tight knit community that offers an abundance of outdoor activities. The area has hundreds of lakes and rivers and a vast amount of rolling hills and forests. The local economy is well diversified, although resource oriented.

Wage Range:

The target hiring range for this position is $31.89 to $41.30 per hour. Candidates will typically be hired, transferred or promoted between the minimum and midpoint of the wage range. Placement within the wage range is dependent on the applicants' role specific knowledge, skills, abilities, related experience and in consideration of factors such as market conditions and internal equity.
