The Role: Family Physician in Toronto

Provide comprehensive primary care with a focus on prevention, chronic disease management, and patient education. Collaborate in an interdisciplinary team to deliver patient-centered, team-based care. Build a patient panel focused on prevention and longevity, emphasizing proactive and personalized care. Leverage technology and innovation to enhance patient care, including POCUS, mental health technologies, and genetics. Exceptional compensation with blended payment model. Experience in a collaborative, team-based primary care setting and in chronic disease management preferred. Position for New Location Opening Fall 2025 at Yonge & St. Clair

Experience You Bring & Qualifications

Certification in Family Medicine (CCFP) from the College of Family Physicians of Canada (CFPC) Current registration with College of Physicians and Surgeons of Ontario (CPSO) At least three of clinical, primary care experience
