This position is responsible for providing support to adolescents and young adults 12-25 years of age who are experiencing mental health issues. The incumbent will also provide additional supports to Adolescents/young adults (A/YA) to work toward the achievement of their goals as identified in the person's recovery plan and work closely with the primary worker.




Reporting to Manager of Client Services

,

the incumbent will be responsible for the following functions:




promotes a stigma free youth culture for mental illness; conducting and completing the detailed Community Mental Health Assessment; assisting A/YA in prioritizing needs/areas of consideration and to develop short-term plans to address these priorities; promotes opportunities for increased independence and champions community peer development; acting as a resource person or an advocate to assist individuals in obtaining information/access to services/supports which are deemed needed; and supporting reintegration into school, work and social activities;.



Qualifications:




Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the C.N.O. Bachelor's Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or Bachelor's Degree with an applicable college diploma in Addictions and current registration with the C.A.C.C.F. 1-2 years' experience in a health related organization and experience working with Adolescents/young adults with mental health issues; Personal lived experience in recovery from a mental health and/or addiction challenges; A valid Ontario Driver's Licence and access to a vehicle are required.



Hours of Work:

35 hours/week. Evening hours are required and occasional Saturdays may be required to meet the position requirements.

Salary:

Commensurate with experience within the range: $30.82-$43.86 inclusive of HOOPP
