This position is responsible for providing highly individualized and flexible support to adolescents and young adults who are experiencing their first onset of psychosis and/or a mental illness. In addition, the incumbent will engage families, providing education and support to them through the treatment and recovery process.
Reporting to the Manager of Integrated Client Services, the incumbent will be responsible for the following functions:
Conducting and completing detailed InterRAI assessment and where required a health assessment and gathering collateral information for individuals referred;
Develop, implement and evaluate individualized treatment plans with clients on an on-going basis
Recognize the need to escalate acute care needs to the most responsible psychiatrist.
Engaging, educating and supporting families through the treatment and recovery process utilizing individual and group intervention strategies;
Acting as a resource person/advocate in obtaining community services.
Supporting reintegration into school, work and social activities;
Assessing both physical and mental health and liaising with physicians
Qualifications:
Education required:
Registered Practical Nurse Diploma from a recognized post-secondary institution with a current registration with the College of Nurses of Ontario.
Experience and a detailed knowledge of serious mental illness, usually gained by several years of working with this population.
The candidate will work with other professionals using strong problem solving and communication skills.
A valid driver's license, and use of a vehicle, which will be used in conducting agency business, is required.
Skills required:
Must have strong assessment skills;
Must possess knowledge of the developmental stages of adolescents and young adults;
Crisis or Suicide Intervention training is an asset;
CPR training;
Must have extensive knowledge of serious mental illness and trends in the provision of support services to adults with serious mental illness;
Extensive knowledge of the Mental Health Act, as well as relevant legislation specific to individuals with a serious mental illness;
Planning and organization skills to facilitate advocacy and support coordination/service access;
Extensive knowledge of the support/treatment services network in Chatham Kent and of regional services commonly accessed by the program;
Extremely well developed written and verbal communication skills;
Working knowledge of computers with skill in the use of email and Microsoft Office;
Proficiency in both official languages will be considered an asset.
Hours of Work: 35 hours/week with working one evening per week.
