We are currently seeking an

Education & Support Coordinator

who is passionate about providing meaningful support to care partners and individuals living with dementia. This role is ideal for a clinically minded professional who thrives in a mission-driven, collaborative, and client-centered environment.

Position Summary



Reporting to the Director of Programs (or designate), the Education & Support Coordinator provides therapeutic and psycho-social support to care partners of people living with dementia through individual and group-based interventions. The role also includes outreach, intake, and collaboration with local and regional programs to ensure continuity of care. Clinical support for persons living with dementia is required.

This is an in-person position with occasional flexibility for remote work as business needs allow.

Key Responsibilities



Clinical & Program Delivery



Assess, plan, deliver, and evaluate individual and group support sessions for care partners Administer standardized mental health assessments to evaluate outcomes Deliver therapeutic interventions to enhance caregiver capacity and emotional well-being Maintain up-to-date, confidential client records in accordance with legislation, policies, and professional standards Support client intake, referrals, system navigation, and follow-up

Education, Outreach & Collaboration



Facilitate educational and skills-based training programs for family and professional caregivers Conduct outreach to engage and recruit care partners into available programs Build and maintain collaborative relationships with community and regional partners Participate in cross-agency initiatives and knowledge exchange efforts to improve dementia care

Program Evaluation & Administration



Collect and report both quantitative and qualitative program data for internal evaluation and reporting Contribute to quality improvement and planning processes Assist in policy and resource development related to caregiver and client support

Other Duties



Participate in the on-call rotation for emergency coverage related to Adult Day and In-Home Respite programs Perform other duties consistent with the classification, as required

Qualifications & Requirements



Post-secondary degree or diploma in Nursing (RN/RPN), Social Work, Social Service Worker, Occupational Therapy, or Speech-Language Pathology. Master's degree preferred. Current registration in good standing with the

applicable regulatory body

(e.g., CNO, OCSWSSW, CASLPO).

REQUIRED

:

Minimum of 3 years' experience in senior care*, with a strong background in working with individuals living with Alzheimer's disease or other dementias.
Experience managing complex or chronic health conditions is preferred. Valid driver's license and access to a reliable vehicle. Current First Aid, CPR/BLS, and Food Handler certifications (or willingness to obtain). Vulnerable Sector Police Check required. U-First!, Gentle Persuasive Approaches (GPA), and DementiAbility training are strong assets.

Key Competencies



High emotional intelligence, empathy, and a client-centered care approach. Skilled in facilitating therapeutic sessions and caregiver education. Strong interpersonal, verbal, and written communication skills. Ability to conduct psycho-social assessments and risk evaluations. Organized, resourceful, and adaptable with strong critical thinking skills. Solid understanding of dementia care models and community resources.

Working Conditions



Generally working 8:30 a.m. - 4:30 p.m. with occasional evenings for client and care partner education or group facilitation.

Active work environment involving standing, bending, and light lifting (up to 25 lbs).

Local travel throughout Windsor and Essex County.

Occasional evening or weekend hours depending on program needs.

Use of PPE and adherence to infection control protocols as needed.

Annual flu shot and Covid booster strongly recommended.

Compensation & Benefits



Approximately 30 to 37.5 hours per week based on client need

Hourly Rate:

$29.00-$31.00

Enrollment in the Healthcare of Ontario Pension Plan (HOOPP) effective day one of employment.
