As a Dietitian in this community, you would be a frontline professional within Nutrition Services reporting to the Manager, Nutrition Services, South Zone. You will be responsible for assessing, developing, implementing and evaluating client/ patient care plans for the population served by Alberta Kidney Care (AKC) South as well as in the acute care practice area in Lethbridge. The AKC South portion of the position includes working with chronic kidney disease, peritoneal dialysis, and hemodialysis patients. Services are provided in a multidisciplinary setting, as one-on-one appointments in-person and virtually in Lethbridge, Fort Macleod and Crowsnest Pass communities. The acute care portion of this position includes working on medical and surgical units at Chinook Regional Hospital with adult clients. In this position you will work with multidisciplinary teams in ambulatory care in the community clinics listed above and in acute care at the Chinook Regional Hospital using a patient focused approach which includes nutrition counseling and behavioral change theories/strategies as appropriate to achieve nutrition and health outcomes.
Description:
As a Dietitian I, you may provide nutrition expertise to patients (clients, residents) in various AHS facilities and community settings to achieve nutrition and health outcomes. You may be responsible for nutrition assessment, development, implementation and evaluation of nutrition care plans. You may plan and provide public health nutrition services and education programs to promote healthy living and disease prevention for clients/groups in facilities or community settings. Nutrition education is provided to students and other healthcare professionals to support delivery of evidence based nutrition information.
