Edmonton Southside Primary Care Network - Registered Dietitian



The Edmonton Southside Primary Care Network was the first PCN in Alberta. Over the past 17 years, the ESPCN has grown to become the largest PCN in Edmonton and consists of a team of health care professionals who are passionate about creating healthier communities. Position Overview:



We are currently looking for a part-time (0.80 FTE), Dietitian to join our team! The Primary Care Dietitian will provide comprehensive nutrition services and care to patients within the Primary Care Network. The Primary Care Dietitian will assess, plan, implement and evaluate nutrition care plans to patients of all ages. The Primary Care Dietitian will be an integrated team member working to their full scope of practice within the primary care setting.

The Primary Care Dietitian will work with the family physicians in the community, other health professionals and local and regional programming in a multidisciplinary environment to ensure a high quality of evidence-based health care to the practice population. One evening shift (12-8pm) a week may be required.

The Primary Care Dietitian will report to a Primary Care Manager.

Key Roles and Responsibilities:



To work collaboratively in a patient centred, integrated team based primary care practice setting.

To participate in the care of individual patients, practice populations and associated communities.

To conduct nutrition assessments, initiate and implement nutrition care plans and provide appropriate follow-up, according to care plans.

To support and educate patients and their caregivers to towards optimal nutrition to achieve long term health benefits.

To make professional, competent and autonomous decisions with patients as a member of the multidisciplinary team.

To act as a nutrition resource to other multidisciplinary team members, healthcare institutions and community agencies or programs.

To ensure accurate, objective and timely clinical documentation in the appropriate medical record.

Experience, Skills & Qualifications:



Baccalaureate degree with a major in Foods and Nutrition, completion of recognized internship education program.

Current Registration with College of Dietitians of Alberta

Current Basic Life Support (BLS) for Healthcare provider

Vehicle/license - when appropriate

Previous experience:

Primary care setting

Electronic Medical Record

Group facilitation/instruction

Health behaviour change methods (eg Motivational Interviewing)

Understand the Alberta privacy laws and act according to those laws.

Should behave responsibly, be accountable and have excellent written and verbal skills, problem solving/critical thinking skills.
