Family and Client Care Coordinator \xe2\x80\x93 Must be available to work every 4th weekend and occasional weeknight evenings/overnights \xc2\xb7 Provide education, information and counselling support to caregivers of clients. \xc2\xb7 provide nursing care during overnight and daytime hours \xc2\xb7 provide assistance with activities for the clients attending the Saugeen Central Adult Day Program/Owen Sound Day Program \xc2\xb7 must have a valid driver\xe2\x80\x99s license, access to a reliable vehicle, \xc2\xb7 Minimum of three to five years\xe2\x80\x99 experience working with the elderly and the physically challenged and their families. Knowledge of Alzheimer Disease and related disorders, and previous experience working with volunteers is required. \xc2\xb7 Responsible for the intake and assessment of new clients for our community based support services, introducing new clients and their family members to the programs of HCSS Grey-Bruce \xc2\xb7 Current registration with the College of Nurses in required, as is awareness of community service delivery. \xc2\xb7 Will work every 4th weekend in our Overnight Respite Program as well as hours in our Adult Day Program during the week. -This position will work out of the Hanover Day Program primarily for approx 15 months and will transition into a travel nurse for Day Away Programs across Grey Bruce with paid mileage.
