Data Management Coordinator:



Part Time 0.4 FTE (Full Time Equivalent)

o 14 hours/week, specific days to be negotiated with successful candidate

o Primary location of work: Kincardine FHT Main Site, Kincardine, ON

o Role will involve travel between sites and partner organizations

The Kincardine Family Health Team (KFHT) is a group of allied healthcare professionals with a mission to provide comprehensive patient care utilizing an interdisciplinary approach that is responsive to the needs of the community.

We are seeking an experienced and qualified individual to join our team in the role of Data Coordinator, reporting to the Executive Director and Clinical Lead and working closely with our Quality Improvement Decision Support Specialist (QIDSS). The Data Management Coordinator will play a critical role in ensuring accurate and timely reporting to Ontario Health and project partners including the Grey Bruce Ontario Health Team (GBOHT). This position involves coordinating reporting requirements supporting the building of efficient electronic medical record (EMR) queries for data collection, and conducting regular audits to maintain data accuracy and integrity.

Roles and Responsibilities:

- Works with the Executive Director, Clinical Lead, and partner organizations to coordinate the collection of data from KFHT and partnering organizations to fulfill the reporting requirements to Ontario Health related to Expanding Interprofessional Primary Care.

- Support QIDSS with building EMR queries to collect measurement information.

- Support the preparation and presentation of detailed reports for regular meetings with partners and Ontario Health.

- Implement best practices for data management and auditing processes to identify and rectify any discrepancies or data quality issues. Address any queries or concerns from partners related to data reports.

- Participate in working groups and committees related to expanding Team-Based Care and regional user groups to develop data management best practices.

- Liaise with GBOHT digital lead regarding implementation of new or pilot initiatives.

- Assist with Quarterly Reporting for Ontario Health (EMR searches and OH reporting template).

- Online appointment booking set up for providers and reporting to OH.

- Provide support and coaching to KFHT providers to maximize the effective and efficient use of the EMR (Telus PS Suites and Accuro) and related patient engagement software (ie. Ocean).

- Development and maintenance of EMR forms, toolbars, and documentation.

- Manage EMR and Ocean user access for KFHT employees and provide orientation for new employees.

- Assist FHT staff in collecting and interpreting data / outcome measures as a part of decision making.

- Leverage resources within EMR and patient engagement software to introduce relevant tools and technology.

- Complete privacy audits at the direction of the Privacy Auditor

Specific Education / Job Requirements

- Undergraduate degree in a relevant discipline (Health Informatics, Data Management, Information Technology, Health Sciences, Statistics, Business Administration) or equivalent experience and education.

- Two or more years of related experience, preferably in a health setting.

- Proficiency in electronic medical record systems (such as Telus PS Suite, Accuro), data querying, and reporting tools.

- Strong analytical skills with a keen eye for detail and accuracy.

- Excellent verbal and written communication skills and a proven ability to develop relationships with partners.

- Previous experience in handling confidential or sensitive information, including Personal Health Information.

- Valid Ontario Driver's License and access to a reliable vehicle.

o Port Elgin Family Health Network (Dr. Earl Health Centre), 786 Goderich St, Port Elgin, ON

o Saugeen Shores Family Health Organization (Saugeen Shores Medical Building), 36 Grey St N, Southampton, ON

Compensation: $38.59/hour - $41.84/hour
