Under the direction of the Executive Director the applicant should have a background in health care, social work or a related field. Knowledge of Indigenous cultures, traditions and health practices. Strong communication and interpersonal skills. Ability to navigate the healthcare system and advocate for Indigenous individuals. Understanding the social determinants of health within Indigenous communities.
Duties
Provide day to day health system navigation support for Indigenous clients
Bridge the gap between Indigenous peoples and health care services
Coordinate client referrals to and from interagency, Indigenous communities and health care services
Act as a referral access point and perform general intake assessments for individuals.
Support Indigenous patients in and out of the hospital, addressing their concerns and providing follow up care
Awareness and ability to teach Indigenous knowledge systems to AHS staff
Requirements
Strong educational background in health sciences, social work, or a related field.
Excellent communication skills, both verbal and written, with a demonstrated ability in public speaking.
Experience working with Indigenous organizations and clients
A compassionate approach with a commitment to advocating for clients' health needs.
Ability to work independently as well as collaboratively within a team environment.
