The Coordinator provides an integrated patient-centered approach to care that focuses on enhancing and coordinating the care for patients living within Couchiching area.



The goal of the Coordinator is to create seamless care coordination for patients. The care coordinator will ensure that each patient has a Coordinated Care Plan (CCP) and ongoing care coordination. The coordinator is responsible for working with home and community care to assess the patient's eligibility for in home services and ensure the appropriate documentation is submitted to the CHRIS system.



The coordinator is responsible for the continued evaluation and monitoring of the patients health status, need for healthcare services (including home services) and eligibility for additional community and home support services in collaboration with home and community care.



PRIMARY RESPONSIBILITIES:

Develop a coordinated care plan for each patient assigned, facilitate the implementation of the multi-disciplinary service plan to meet the patient's needs in the community Provide navigation for patients and families, including linking patients to community and health service resources. Assess, negotiate and problem solve. Utilize effective interpersonal communication skills, demonstrating mutual respect for all involved parties Ensure the fiscally responsible use of appropriate resources to achieve the desired outcomes by mobilizing and integrating formal and informal patient support networks. Effective link patients and families with appropriate resources. Collaborate with hospital and community teams to promote and plan for early discharge Engage patients and families in information sharing Complete comprehensive patient assessments, using validated assessment tools. Performs a through assessment including patient's goals for care, physical and emotional health, cognitive status, coping abilities, economic resources and environmental conditions. Communicate with patients, families and other members of the healthcare team Complete all documentation as required Meet with patient/family at regular intervals including in all relevant care settings.

Qualifications




Education:

A regulated healthcare professional (required)



Experience:

Minimum of two (2) years in a leadership role Acute care experience required Community care experience preferred Experience working with different patient populations
