Title: Coordinator- Priority Assistance to Transfer Home (PATH)

Employee Working Location: Office / In-person (Sudbury, Ontario)

Employment status: Temporary full time (12 months)

Salary band: $42,889-$53,611/year

The incumbent will work in collaboration with Sr. Manager, the PATH Coordinator will coordinate and implement all aspects of the Priority Assistance to Transfer Home (PATH) program. The PATH Program is a community program designed to facilitate a smooth transition from hospital to home and ensures that community support services are in place for clients.

In this role, you will:

  • Develops, coordinates, implements and evaluate the delivery of the PATH program.
  • Participates in client discharge planning (hospitals/community support services).
  • Approves referrals, complete intakes, and determine internal or external care coordination.
  • Schedules request for service/ transportation and confirm referral acceptance back to the participating hospital.
  • Ensures staff follow safety protocols and policies of the agency and stakeholders.
  • Follow up with PATH clients within 48 hours after service delivery to complete the service, assess for additional services, refer to other services as appropriate, and conduct client satisfaction surveys.
  • Assists with preparing and monitoring program specific budgets, complete program statistical reports and assist with preparing progress reports.
  • Establishes the qualitative and quantitative data required to evaluate the program in consultation with hospital, HCC, and participating agencies.
  • Plays an active role in identifying, developing, and implementing new or re-aligned services/initiatives.
What we are looking for :
  • One to three year\xe2\x80\x99s community college diploma in Gerontology or related education OR one to three years job related experience OR an equivalent combination of education and experience.
  • Fluency in French and English is required.
  • Standard First Aid and CPR-C.
  • Knowledge of community support services Safe Food Handling training.
  • Professional documenting.
  • Valid drivers license and access to a reliable vehicle with the ability to travel in the community.
  • Proficiency in Microsoft Office applications (Word, Outlook, Excel, Power Point) is required.
  • Good communication/presentation skills, leadership abilities, and experience working with clients and volunteers is essential.
