Coordinator, Health Navigation & Disability Support



(Short-term contract to June 27, 2026)



ISANS is seeking a full-time Coordinator, Health Navigation & Disability Support to support newcomers in accessing information and services important to their health and wellbeing. The Coordinator will also support and educate service providers and advocate for culturally sensitive, inclusive and responsive services to newcomers. Reporting to the Supervisor & the Manager, Refugee Resettlement, this position is responsible for:

Ensures effective development, delivery and evaluation of the Immigrant Health Program by:



Providing case-management based services including initial and ongoing assessment of client's health related goals and health needs; Providing information and referrals to appropriate services, linking clients with health care providers, both in health care systems and/or community based health service providers. Supporting clients with disability and complex health needs to access specialized services and obtain necessary medical equipment, mobilizing urgent services when needed. Assisting clients in navigating health care system and providing problem-solving support and guidance with issues related to the health system or specific health conditions Advocating and consulting with service providers on behalf of the clients hen appropriate. Organizing and coordinating educational orientation sessions about Canadian Health Care System and sessions on other various health related topics, in family and/or group settings. Educating service providers and raising awareness of issues affecting health of government assisted refugees, providing information about federal systems supporting refugees (e.g. Interim Federal Health Program, Resettlement Assistance Program). Collaborating with service providers to determine and establish referral processes, overcame identified gaps in service delivery and build strong and sustainable partnerships for consistent, coordinated and effective service provision. Participating in strategic initiatives, working groups, committees or other bodies on developing and building strategies and intervention that address an promote refugee health issues Participating in ISANS initiatives and activities related to immigrant health and disability in coordination with other ISANS teams.

Develops the Settlement Support System by:



Maintaining accurate registry of our health care service partners Providing information about the ongoing needs of clients to health professionals and funders Participating in developing documents, proposals and reports for existing and/or new projects Sharing Refugee Resettlement Team Programs' coverage duties

General responsibilities:



Performing other duties as required Providing information as required for internal/external reports, proposals, grants etc. Reviewing and adhering to ISANS workplace health and safety policies; safety is everyone's responsibility Recording all relevant client information on databases in a consistent and timely manner as appropriate to the position Attending regular Staff Meetings and Team meetings Attending training and development opportunities as appropriate Attending and providing support at ISANS events

The ideal candidate for this position will have the following:



Education:



Bachelors Degree in Social Work/Health related Equivalent combination of education and experience will be accepted

Experience:



2 years experience in a similar position Experience working in a cross-cultural environment; experience delivering direct service to immigrants preferred Experience developing, implementing and evaluating programs or projects Experience delivering service face to face and/or online Experience with networking and outreach Experience in group facilitation

Knowledge:



Knowledge of local activities, programs and community organizations that serve the immigrant population

Skills:



Strong written and verbal English communication skills Ability to work independently and as part of a team Strong time management and organization skills Proficient with MS Office applications, Databases and other systems Additional languages an asset

Terms of Employment:



Short-term contract to June 27, 2026 Full-time 35 hours per week Hybrid work environment: work location will be determined by operational requirements, prioritizing ISANS mission and clients to determine the right balance of in-person and virtual work.
