Queens Park Care Centre (QPCC)

is located in New Westminster and provides rehabilitation services, an adult day program and a long-term care community. QPCC has an outpatient rehab service and 76 specialized inpatient rehabilitation beds. These provide intensive rehab for different patient populations including Brain Injury, Spinal Cord Injury and Stroke rehab patients as well as lower leg amputees, complex neurological conditions and other muscular skeletal injuries. Detailed Overview:

Responsible for planning, implementing, coordinating, supervising and evaluating the care and service delivery for the Day Program for Older Adults for an assigned location in collaboration with the interdisciplinary team; schedules and coordinating staff work assignments and acts as a resource to staff, families and clients; assists with the supervision of client care, provides learning opportunities for staff and perform direct patient care, as required.


Responsibilities:
Coordinates all aspect of care and service delivery to meet the client's needs such as maintaining a waitlist of applicants, admitting, discharging clients and developing care plans for individual clients in collaboration with the interdisciplinary team; performs direct patient care such as administering medication, application of treatments and performing client assessments.

Supervises, schedules and coordinates work assignment of staff including vacations, leaves of absence, relief staff and staff rotations to meet program requirements; participates in the recruitment and selection of staff including staff orientation, training, evaluation and discipline, as required; provides direction to volunteers of the program.

Provides input into the preadmission, admission and discharge planning of clients in the Adult Day Program, in collaboration with the Manager, Clinical Services; maintains and keeps pertinent records and charts for each client; assists in the development of forms and maintains daily attendance records; prepares special reports related to the Program, as required. Performing nursing care by observing and recording symptoms and conditions of the client to appropriate staff such as the physician; provides information to the interdisciplinary team regarding client symptoms, reactions and progress.

Assigns designated staff with the provision of basic care such as toileting, feeding, and other activities of daily living; ensures client standard of care is established and maintained; assesses the effectiveness of the program according to the needs of the client; forwards observations and/or recommendations for improvement to the Manager, Clinical Services.

Attends and/or participates in staff information meetings and presents client assessments; recommends appropriate intervention and participates in the development of the client care plan with the interdisciplinary team.

Provides guidance, support and instruction to the client and/or family regarding the delivery of care.

Provides input into the development and implementation of Day Program for Older Adult policies and procedures, client care audits and continuous quality improvement activities, communicates new and revised policies and procedures regarding service delivery to designated staff, and holds regular interdisciplinary team conferences.

Participates in the development of the program budget and provides recommendations regarding the budget to the Manager; monitor usage and effectiveness of supplies and equipment; orders supplies as required and reports malfunctioning equipment to the Manager; provides input into equipment supply requirements for the program. Ensures Client Incident Reports and follow up documentation are completed and forwarded to the Manager, in accordance with established procedures.

Directs safety, fire and evacuation procedures following established procedures, completes necessary documentation as required. Maintains liaison with clinical instructors for external training programs; monitors performance of health care students on practicum and provides information for student evaluations.

Collects, compiles and reports statistical and quality improvement data for research and evaluation purposes.

Performs other related duties as assigned.
Qualifications:

Education and Experience



Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years' recent related clinical experience in a community-based day program for older adults including one (1) year experience in a supervisory role, or an equivalent combination of education, training and experience.
