Your Opportunity:
As the transition coordinator you would be a member of the diabetes multi-disciplinary team and play an integral role in supporting adolescent patients ages 16 to 19 in successful transition from pediatric to adult diabetes care. You will have the opportunity to utilize excellent communication, teamwork, and relationship building skills to support the optimal outcomes for youths with diabetes as they transition into the world of adult diabetes care. Your role will contribute to clinical process improvement and quality improvement initiatives while reporting to and working with the pediatric and adult clinical diabetes teams. As part of the care team, you will collaborate in a positive and supportive working environment and demonstrate accountability to the duties and responsibilities of this role. The role of the Transition Coordinator includes, assessment, planning, and coordination of care, as well as ongoing communication with other health-care providers across the continuum of care. Upon hire, you will receive specialized training providing you the knowledge, tools, and resources to confidently care for these patients. Major responsibilities will include, facilitating medical follow-up appointments and referrals, answering questions, and encouraging engagement of youths with diabetes in their transition of care and diabetes self-management. This role will also facilitate group transition classes in support of youth education and engagement in transition of care. To be successful in this role you will need: provision of patient and family centered care, proficient organizational, technical and computer skills, commitment to professional growth, and excellent communication skills with patients, families, staff, and Physicians.

Required Qualifications:

Additional Required Qualifications:

Bachelor's degree. One year experience working with a multidisciplinary team, including internal and external stakeholders. One year experience leading and facilitating group programs; ability to build teams and navigate conflict scenarios. English proficiency both verbally and written. One year experience with MS Word document development.

Preferred Qualifications:

One year experience as a volunteer or employment with youths (e.g. camp counsellor, youth day programs, youth leader, etc.). PROSCI or equivalent change leadership credentials. One year experience with project management methodology and change management. Experience with SharePoint, Microsoft programs and electronic medical records programs (ConnectCare). Social media and website development and maintenance. Personal or professional experience with diabetes.
