The Truro Housing Outreach Society (THOS), is seeking a strategic, community-minded

System Planner

to lead the development and implementation of a

Coordinated Access System (CAS)

in

Colchester, Truro, and East Hants

.

This position is funded through

Reaching Home - Canada's Homelessness Strategy

and will play a critical role in establishing the structures, tools, and community partnerships required for an equitable, effective, and data-driven response to homelessness across the region.

Key Responsibilities:

System Design & Implementation

Lead the planning, coordination, and development of a new Coordinated Access System in alignment with Reaching Home directives and Built for Zero principles. Facilitate the mapping of local service pathways, referral processes, and assessment tools. Identify and support the establishment of

Access Points

,

Assessment Tools

, and a

By-Name List

system to track individuals experiencing homelessness. Ensure that the system meets federal Reaching Home requirements, including governance, data collection, prioritization, and real-time reporting.
Stakeholder Engagement & Governance

Convene and support a local

Coordinated Access Governance Committee

, including service providers, municipal leaders, and people with lived experience. Act as liaison between THOS, service partners, and other stakeholders across Colchester, Truro, and East Hants. Foster strong relationships and transparent communication between system users, ensuring community ownership and participation.
Data & Performance Management

Oversee the implementation and use of HIFIS (Homeless Individuals and Families Information System) or approved alternative tools. Ensure data accuracy, privacy, and usage in line with Reaching Home and local governance policies. Support continuous improvement through performance metrics, data analysis, and regular reporting to the CAB, community, and federal partners.
Capacity Building & Training

Develop training materials and deliver sessions for staff and partners on Coordinated Access processes, HIFIS, and system protocols. Support onboarding of service providers and Access Points to the new system. Promote trauma-informed, culturally responsive, and equity-focused service delivery through all CAS components.

Qualifications & Experience:



Post-secondary education in Social Work, Public Policy, Community Development, Health Administration, or a related field.

3+ years experience

in system planning, homelessness response, or community-based coordination. Familiarity with

Reaching Home

,

Coordinated Access

, and/or

Built for Zero Canada

models. Experience working with or within diverse and rural communities; understanding of local barriers and strengths in Colchester, Truro, and East Hants is a strong asset. Proficiency with

HIFIS

, client management databases, and data analysis tools. Strong facilitation, project management, and stakeholder engagement skills. Understanding of housing-first principles, harm reduction, and trauma-informed care.

Assets:



Experience working directly with individuals experiencing homelessness or housing precarity. Knowledge of provincial housing systems, health, and social supports. Familiarity with rural service delivery and cross-jurisdictional coordination. Experience coordinating governance tables or collaborative working groups. Understanding of Coordinated Access systems and BNL

Working Conditions:



This is a contract position funded through Reaching Home, with the potential for renewal pending continued funding. The role will require regular travel throughout Colchester, Truro, and East Hants; a valid driver's license and access to reliable transportation are required. The position is primarily work from home with regular office check in's when required Occasional evening meetings or community events may be required.

Pay: $25.00 per hour

Expected hours: No more than 40 per week
