Cluster Care Pilot - Gordon B. Isnor Manor - Nora Bernard Street
The Cluster Care Pilot seeks to positively impact the health of Nova Scotians living in public housing by building on the existing Provincial Homecare Program.
The Cluster Care team will support individuals with additional programs offering physical and social opportunities and help reduce barriers to accessing other continuing care services. For example, you could be supporting someone by accompanying them to an appointment, helping put away their groceries, supporting them with activities of daily living or participating in a social or wellness program with the individual. At times, you may also be assigned clients receiving Homecare as well.
This is a hybrid opportunity where you are able to support older adults to live more in their community. Northwood will be providing community support services for the Gordon B. Isnor Manor; this is a pilot program running for 12 months with a possible extension. The pilot will be set on a rotation including weekends, the hours will be determined by the need from the building and operational requirements, the shifts will be day/early evening.
Flexibility will be required.
Responsibilities:
Provide personal care (e.g., bathing, dressing, grooming)
Perform light housekeeping duties
Meal preparation
Provide respite for families and care givers
Documentation (e.g., reporting client concerns, progress notes, incident reports)
Support recreational/well-being activities and programs
Accompany client to medical or other essential appointments
Provide services to clients as per individual care plans developed by Supervisor
Completes other tasks as required.

We are looking for a dynamic individual to be a part of our
Home Support Cluster Care
team for the position of
Continuing Care Assistant (CCA).
Qualifications:
Completion of a recognized CCA program.
Provincial CCA certification.
Home Support Basic Training Certificate
Current Standard First Aid/CPR
Ability to communicate effectively, both verbally and in writing, with clients, families, visitors, and coworkers.
Good organizational skills and a strong customer service (client/ family) focus required.
Ability to work well with others.
Ability to attend work on a regular and consistent basis according to the Northwood Corporate Standards.
Demonstrated use of proper body mechanics and ability to perform heavy lifting.
Knowledge of / experience working in a team-based, person-centered approach to care.
Demonstrated ability to work effectively as a member of an interdisciplinary team.

Salary /Benefits:

Hours of Work: 80 hours bi-weekly
