About the Opportunity
Reporting to the Manager, Planning & Development, the Operations Consultant (Practice Support Programs), is responsible for working with the broader PHC team to advance the strategic and operational priorities of the portfolio. The Operations Consultant (Practice Support Programs) has a responsibility for supporting the overall coordination of the planning, development and implementation of the activities assigned under the direction of the Senior Director and Senior Medical Director for the Primary Health Care and Chronic Disease Management Network.
The scope of practice will include various roles as facilitator, coordinator, communicator, and planner. They work according to the mission, vision, values and policies of NS Health and the Primary Health Care and Chronic Disease Management Network to support the development, implementation, and evaluation of PHC practice support and capacity building programs, services and initiatives throughout the province. The Operations Consultant\'s activities are guided by PHC priority areas, multiple stakeholders and by community needs and, therefore, require a professional approach. This work requires recognition and application of best practice, evidenced informed decision making and practice facilitation to build capacity and enable practice change / practice improvement. The Operations Consultant, Practice Support Programs, will have areas of focus related to integration of a palliative approach to care within primary health care, as well as other priorities as identified by the Network, clinicians, physicians, and leaders in PHC.
