Position Summary:



The Concurrent Disorders Case Manager provides specialized case management services to individuals experiencing co-occurring substance use and mental health challenges. Reporting to the Program Manager, the Case Manager works collaboratively with hospitals, healthcare providers, social services, and community partners to ensure a holistic, person-centered approach that reduces barriers to care, supports recovery, and enhances overall well-being. This role is responsible for assessment, intervention, service coordination, and follow-up for individuals aged 16 years and older. The Concurrent Disorder Case Manager carries a caseload of 18-25 clients, unless otherwise directed, and provides services in alignment with harm reduction, trauma-informed care, and recovery-oriented principles.

Qualifications & Experience:



College diploma or university degree in Social Work, Psychology, Addictions Counselling, Nursing, or a related field, or an equivalent combination of education and experience. Minimum of 3 years of experience working with individuals experiencing concurrent disorders, homelessness, and substance use challenges. Certification in First Aid and CPR. Applied Suicide Intervention Skills Training (ASIST) and Non-Violent Crisis Intervention Training (NVCI) is an asset. Experience working within a case management model is preferred. Strong knowledge of mental health and substance use disorders, harm reduction, and trauma-informed care. Ability to assess and intervene in crisis situations using de-escalation and conflict resolution techniques. Demonstrated knowledge of community resources, social determinants of health, and systemic barriers. Knowledge of and ability to apply legislation such as the Mental Health Act, Health Care Consent Act, and PHIPA. Excellent problem-solving, prioritization, and conflict resolution skills. Ability to work autonomously and as part of a multidisciplinary team. Strong advocacy and system navigation skills to support client engagement in care. Proficiency in Microsoft Office (Word, Excel, Outlook, Teams, SharePoint). Valid Ontario Driver's License and access to a reliable vehicle for community-based work. Satisfactory criminal record check and vulnerable sector check required prior to hire. Availability to work flexible hours, including days, evenings, and weekends as required.
