Established in 1993, Y Newcomer Connections is a leading immigrant settlement Centre focused on creating connections that will transform our community into a lasting, healthy home for immigrants from around the world and from all walks of life.The OpportunityAs a Complex Case Worker at The YMCA of Southwestern New Brunswick, you\xe2\x80\x99ll be a vital advocate and support for newcomer clients, particularly Government Assisted Refugees (GARs) and Permanent Residents with complex needs. In this role, you will coordinate health services, navigate government programs, and provide specialized support to help clients overcome challenges during their settlement. You\xe2\x80\x99ll work closely with healthcare providers, arrange interpreters for clients with language barriers, and ensure urgent health needs are addressed. Your efforts will help bridge gaps in service delivery, offering comprehensive guidance to clients while fostering relationships with community organizations and volunteers. This role requires flexibility for evening and weekend work and reports to the Manager of Settlement and Resettlement Support Services.Responsibilities

  • Conduct assessments to determine strengths and needs of high-needs and complex case Government Assisted Refugees (GARs) and other Permanent Residents (PR), creating and implementing tailored case plans.
  • Provide supportive counseling, referrals, and information to help clients overcome settlement challenges.
  • Assist clients with navigating government systems and completing necessary documentation for successful settlement.
  • Coordinate initial health check-ups and urgent healthcare referrals for GARs, ensuring timely access to medical specialists.
  • Collaborate with Health Working Groups to address service gaps in areas such as Community Health, Public Health, and Mental Health.
  • Guide clients and healthcare providers through the Interim Federal Health (IFH) program and connect clients to Post Arrival Health Assessments (PAHA).
  • Work with the Health Liaison to support clients with settlement needs, including healthcare and government programming.
  • Schedule health appointments for clients with complex needs, ensuring appropriate care for disabilities and mobility issues.
  • Follow up with families facing health or complex settlement challenges to ensure ongoing support.
  • Deliver information sessions on healthcare resources, prescriptions, and emergency contacts for clients.
  • Build relationships with local healthcare providers, pharmacies, dentists, and opticians to enhance client support.
  • Provide resources on health and well-being, including mental health support.
  • Support Newcomer Connections staff during busy periods and share insights on healthcare needs within the newcomer community.
  • Maintain accurate client records, manage expenses, attend staff meetings, and perform other duties as assigned.
Skills and Qualifications
  • Bachelor\xe2\x80\x99s Degree, Community College diploma, or equivalent from an accredited institution.
  • Minimum 2 years of experience in a cross-cultural environment, particularly with immigrant or refugee communities.
  • Fluent in English; knowledge of other languages is an asset.
  • Exceptional communication, organizational, and interpersonal skills.
  • Demonstrated success in building cross-sector relationships
  • Flexibility to work evenings and weekends when necessary.
  • Proficiency in MS Office 365 and client management databases.
  • Strong understanding of the needs and challenges faced by newcomers in Canada.
  • Employment is contingent on having a clean Criminal Record Check.
