Key Duties and Responsibilities:
Participates in the assessment, goal setting, and program planning such as Support Plans for individuals with support needs. Documents and implements the plan. Provides input into the evaluation of the program, in consultation with the program manager.
Evaluates participant's needs and develop short-term plans to meet such needs with families.
Support participants in working towards having independence or semi-independence in the community, by assisting participants with daily life skills, social skills, and/or behavior management. Teaches and assists participants with activities such as personal hygiene, basic cooking, money management, shopping, and household safety. Facilitates physical, recreational, educational, social, and vocational activities.
Assists with the intake process to ensure assessment, program registration, and all documentation is completed prior to the service beginning for assigned programs.
Recognizes, analyzes, and deals with potential emergency situations such as participant's aggressive behavior to minimize potential harm to the participants and/or the public. Reports problems to the supervisor.
Administers medication to clients in accordance with established policy.
Ensure health and safety standards are maintained.
Provides feedback and support to participants and their families.
Maintains written reports regarding participants' daily activities and progress. This includes progress reports, transition reports, and daily case notes. Ensure that all required documentation is complete, accurate, and up to date.
Transport participants from school sites to club sites, or out trips.
Performs other related duties as required.
Qualifications:
Diploma in a related human/social service field, one (1) year recent related experience, or an equivalent combination of education, training, and experience.
Class 4 driver's license or willingness to obtain as soon as possible.
Successful completion of Agency screening process including criminal record check.
