The Caldwell Family Centre is looking for a dynamic, compassionate, and versatile Community Support Coordinator to assist clients with a wide range of assistance requests. This position also contributes to community development through event planning and collaboration with community partners.

Job Purpose

:

The purpose of the Community Support Coordinator is to provide WrapAround services that are client-centric to reduce barriers and improve the quality of life by helping each individual reach their self-identified goals in a holistic, empowering and personalized way. This position plays a critical role in establishing formal pathways, and developing partnerships with health, social and community support programs based on identified community needs.

Overview:



This position will be working directly with community members who are low-income or living in poverty who face multiple barriers, to assist them in finding the support they need to meet their challenges. The Community Support Coordinator will work directly with clients to provide assistance, or direct them to support in areas such as housing, practical assistance (job search; online forms), crisis intervention, linkages to primary and mental health care, and a range of other services based on their needs. A key component of this position is building a support network of community resources that will enable the individual to increase their independence, dignity, and self-respect.

Responsibilities include, but are not limited to:



Meeting with clients who seek out the services, or are referred by other staff members, to identify their specific needs Identifying clients who may need support services by participating in meaningful engagement at the Drop-In or other programs Conducting a thorough client intake Assisting clients in the development and implementation of a plan tailored to their needs and issues designed to improve quality of life Making appointments for clients as necessary with the Mental Health Nurse, and ESS Worker Acting as a liaison and client advocate amongst those involved with the client's care including family, other caregivers, medical and other multi-disciplinary providers Interacting with clients, staff, family members, other community agencies and partners in a professional manner that reflects favorably on the organization Providing intervention de-escalation services along with a calming influence during minor incidents involving clients Keeping accurate records of interaction with clients, indicating type of action/engagement (housing support; mental health referral; assistance with job applications, etc.) Executing the Computer Cafe, Drop-In Centre, Backpack and Holiday Assistance Programs Ensuring all program reports required by the Executive Director and funders are completed and submitted on a timely basis Overseeing the collection and maintenance of client records for statistical purposes per the confidentiality and privacy policy of the Caldwell Family Centre and funders Attending meetings of other similar organizations as appropriate, to gather best practices Identifying and liaising with partner and potential partner organizations for client services Supervision and training of students and volunteers assisting with the program Assessment of students in field placement positions as per the educational institution's requirements Liaising with other groups using the facilities, maintaining the booking calendar Working with the Program Committee to facilitate client surveys and group activities such as BINGO Assisting as needed in other program areas

Qualifications:



Completion of degree in social services/human services or equivalent Minimum 3 years of experience in providing support to the client population including those with mental health, health &/or addiction challenges Knowledge and ability to work clients in addictions recovery, and an ability to come to an agreement with clients on goals/priorities Crisis intervention skills, with well-developed problem-solving abilities and interpersonal skills Good oral and written communication skills and computer literacy, including MS word and Excel Knowledge of community resources and supports Ability to work effectively as part of a team and interact with workers from a variety of agencies Creativity and flexibility to approaches Bilingual French/English. Knowledge of other languages is an asset. Understanding of cultural competency and anti-oppression framework Experience working with Newcomers to Canada Ability to work as a team player as well as on own with minimal supervision
Job Type: Full-time

Pay: $22.00-$24.00 per hour
