Community Support Worker (CSW) who will provide support in programs that support adults with mental health disabilities and addictions and housing issues.
Job Duties:
Provide day-to-day service support including budgeting, medication support, and issue resolution.
Facilitate referrals to appropriate community services and support.
Respect the autonomy and dignity of clients in all interactions.
Collaborate with clients to understand their goals and provide support aligned with their direction.
Liaise with community partners to build strong relationships and opportunities.
Participate in implementing mental health service strategies and facilitate client access.
Maintain clear communication with the Program Manager and coworkers via daily logs, client updates, and communication boards.
Requirements:
Post-secondary education in Social Services or Health Sciences (e.g. Registered Practical Nurse, Personal Support Worker, Disability Support Worker, Social Services Worker, Community Support Worker, Bachelor's Social Work, etc.).
1-2 years of experience working with clients with mental health or substance abuse issues.
Must be able to work every other weekend.
Ability to work independently and collaboratively within a team.
Commitment to confidentiality and ethical handling of sensitive information.
Valid Ontario driver's license, daily access to a motor vehicle, and appropriate vehicle insurance coverage.
Clear Police Clearance.
