We are currently seeking a Community Support Worker - HASP (Housing and Support Peel) to join our team.
The incumbent will link with and assist persons with serious mental illness who are homeless or at risk of homelessness and who are eligible for referral to Phase II funded units from Supportive Housing in the Province (SHIP); once the client is housed, the worker will provide the necessary community support in order to maintain housing. Additionally, they will provide discharge planning services and assist in the reintegration/integration of recently released individuals into their home community or community of choice.
Status:
Contract full-time until March 2026, 37.5 hours per week.
Salary:
$61,201.01 - $72,001.19 annually (Candidate qualifications will be considered when determining the starting salary)
Location:
This position will be based out of the 60 West Drive, Brampton, Ontario office, with opportunities for occasional remote work.

What you will do
Provide case management services to a case load of clients
Assess the client's need for supportive housing, and their eligibility for access to units provided within the Housing and Support Peel Program
Facilitate the client's move into their supportive housing unit, as needed
Establish appropriate rehabilitation goals and assist the client in reaching them
Provide crisis intervention, when necessary
Refer and link clients to appropriate programs
Provide ongoing supportive counseling and case management, when appropriate
Provide support specific to life skills, daily living skills, conflict resolution, crisis intervention and prevention, budgeting assistance, community integration and community building
Educate community agencies and institutions regarding mental health discharge planning services.
In collaboration with institutional staff, develop relevant policies and procedures for the discharge planning services.
Conduct case conferences/consultation with treatment providers, including physicians, other community organizations and other significant persons, as deemed appropriate
Establish and maintain working relationships with Institutional Social Workers, Probation Officers, Consulting Psychiatrists, Institutional Personnel, community agencies and schedule 1 hospitals
Respectfully advocate on behalf of clients, maintain current knowledge of various linkages with community resources
Maintain effective and timely documentation and client records
Participate in regularly scheduled team meetings and in regular supervision including annual performance appraisal with the Program Manager
Participate as a full member of HASP team
Provide the court support function to clients as directed
What you need to join our team
University degree or College Diploma in Social Work or Social Services. BSW preferred
Regulated Health Professional designation is an asset
At least 3 years of relevant case management experience is required in a community mental health setting
Some experience required working with clients in the Criminal Justice System
Working knowledge of the Forensic Mental Health System in Ontario, Criminal Justice System, and Recovery principles
Demonstrated ability to perform risk, need and psychosocial assessments
Demonstrated ability to work independently in a multi-task environment and to prioritize tasks in a manner that meets client needs and program objectives
Ability to communicate in both verbal and written form with professionals from a variety of disciplines (i.e. medical, legal
Knowledge of CRMS and OCAN is an asset
Training in ASIST, Non-Violent Crisis Intervention and Crisis Response is an asset
Knowledge of French or any other second language that reflects the diversity of Peel Region is an asset
Current CPR/First Aid certification
Valid driver's license, automobile in good repair and insurance coverage as required by agency policy
Proof of administered full-series Health Canada approved COVID-19 vaccination prior to start date. If unable to be vaccinated for reasons relating to protected grounds under the Code, completed medical or religious exemption must be provided
Satisfactory records search and vulnerable sector screening
