About the Role
We are hiring a full-time Community Support Facilitator to provide one-on-one, brain-injury informed support to individuals across the Central and North Okanagan. This is a hands-on, community-based role that involves outreach, systems navigation, and life skills support.
You will work directly with clients in their communities, supporting goal setting, daily functioning, and access to services. The role includes collaboration with internal team members and external community partners. It is well suited to someone who is adaptable, people-focused, and comfortable working outside of a traditional office setting.
Key Responsibilities
Client Support and Coordination
Provide one-on-one, brain-injury informed support in community settings
Support clients with life skills development and compensatory strategies
Develop, implement, and document individualized support plans
Offer consistent outreach, follow-ups, and check-ins
Meet clients in flexible, accessible locations based on their needs
Systems Navigation and Advocacy
Support clients to access health, housing, income, and community services
Act as a liaison between clients and service providers
Advocate for client needs in a respectful and solutions-focused manner
Promote stability, risk reduction, and positive engagement with community systems
Community Engagement and Education
Build and maintain relationships with community partners
Contribute to education and awareness efforts related to brain injury
Participate in outreach initiatives and community events
Who We're Looking For
Diploma or degree in social work, psychology, community support, or a related field
Experience supporting individuals with brain injury, mental health challenges, or substance use is an asset
Entry-level candidates are encouraged to apply, training and supervision will be provided
Trauma-informed, culturally respectful, and client-centred approach
Strong communication, organization, and relationship-building skills
Comfortable working independently and as part of a team
Class 5 driver's license and access to a reliable vehicle required
Compensation and Benefits
Hourly wage of $25 to $30, based on experience
