Community Outreach Services provides evidence based and high quality mental health support to individuals and families living in in East Toronto. As an essential member of the multi-disciplinary team, the Community Support Counsellor will join the REACH intensive case management service with a primary focus of providing high quality and evidence informed case management, supportive counselling and skill teaching to clients with severe and persistent mental illness in the environment of the client's choice. The Community Support Counsellor will carry a client caseload and work collaboratively with other team members to support client care. The Community Support Counsellor may also offer client groups as appropriate.
Education
Baccalaureate in a related field plus a college diploma in Human Services with a major in mental health
Evidence of ongoing commitment to professional development
Qualifications
Must have a minimum of three (3) years front line experience working in the area of community mental health support.
Demonstrated knowledge of the Recovery Model as well as consumer/survivor issues, required.
Demonstrated knowledge of Mental Health Act, required.
Knowledge and understanding of diverse cultures, required.
Excellent knowledge of housing, social, financial, work and other community resources.
Demonstrated ability to assess, plan, implement and evaluate client needs.
Excellent verbal and written communication skills, required.
Demonstrated effective interpersonal and coaching skills, required.
Flexible, organized and self-directed within scope of responsibility; has initiative and independent judgement.
Demonstrated ability to multi-task, problem-solve with proven critical and creative thinking skills.
Demonstrated ability to work well independently and in a team environment.
Demonstrated computer competence, required.
Demonstrated ability to advocate strongly on behalf of the client.
Satisfactory work and attendance record, required.
