Job Description

In collaboration with the Frontenac, Lennox & Addington Ontario Health Team, Maple Family Health Team is building a new multidisciplinary program to strengthen primary care across Kingston. Providers will support the attachment of patients in the area to primary care by providing dedicated clinical support to family physicians from a centralized care hub.

Position Description:



The Community Service Worker works with patients who have complex needs, advocating for and linking them to community resources that will resolve unmet social health needs such as housing, income security, food security, transportation, literacy and social isolation.

Representative Duties and Responsibilities:

Apply a patient-centered approach to work with complex patients in determining the social health action plan. Apply a therapeutic relationship framework, non-judgmental attitude, warmth, respect and caring with patients and their families. Facilitate social health community-based referrals. Facilitate application processes with government and community services. Liaise with community supports to advocate for patients. Compile information about community services to share with Health Hub providers and primary care physicians. Participate in planning and continuous improvement for Maple's Mental Health program Collaborate with other members of the care team, which includes the referring family physician, Clinical Psychologist, Community Service Worker, and other allied health providers at the Health Hub. Facilitate connections to community resources and support as appropriate. Participate in quality improvement initiatives, team meetings and professional development activities. Mentor and support students in clinical and administrative placements, fostering a positive learning environment. Maintain accurate, timely documentation in the electronic medical record.
Qualifications and skills:

College diploma in Social Services 1-2 years of practical experience A strong understanding of the social determinants of health Awareness and understanding of relevant legislation (e.g. Health Care Consent Act, Substitute Decisions Act, Personal Health Information Protection Act, Child and Family Services Act) Experience and interest in offering occasional group programming Experience working with clients of all ages, particularly adults and older adults Proficiency in electronic medical records and digital tools for care delivery Commitment to patient confidentiality, professional standards and evidence-based practice Possession of a driver's license and a reliable vehicle (asset) Knowledge of community resources within Kingston and surrounding areas (asset) Ability to practice in a second language (asset)
Information about the position:

This is a permanent, full-time (Monday - Friday) position. Anticipated start date: January 2026 All providers in the Health Hub will participate in a rotating schedule for occasional evening availability (one-to-three evenings per month). HOOPP benefits begin immediately. Dental and Extended Health benefits begin after 4 months of employment. Full COVID vaccination is required unless there is a religious or medical exemption. Proof of Measles immunity is required. The maximum compensation for this position is aligned with Ministry of Health funding for Community Service Workers in a Family Health Team, which, for 2025-26 is $62,775 per year.
