Position Title: Community Reintegration Facilitator Full-Time Permanent Position, (Female Home)
Hours: 80 Hours Bi-Weekly
Wage Range: $18.74 - $22.47 per hour
Schedule: Primarily 12 hours, Day/Evening Shifts, Alternate Weekends

Administrative Duties:
Consistently adhere to, enforce and effectively implement all UCHH policies and procedures;
Keep current on all changes in policy and procedures;
Accurately account for the whereabouts of all residents through maintenance of destination logs; circulating and interacting with residents; random phone calls to off-site destination; bed counts during night time hours;
Act immediately if residents violate conditions of their residency, following established protocols;
Maintain vigilance for unusual activities and monitor resident behaviours, effectively communicating observations orally and in writing in a timely manner;
Take initiative regarding safety of staff and residents as necessary;
Conduct resident intakes when assigned, including completion of forms; orientation; itemizing, searching and storage of incoming materials;
Participate in shift briefings; accept documented handover of any monies and materials as required; read, initial and date reports within first 60 minutes of shift;
Ensure daily schedule is followed including appointments, meal times, and specific curfew checks;
Complete room searches as directed, secure any contraband found and follow notification procedures;
Conduct emergency drills and regularly check and report any maintenance or safety issues;
Accept deliveries such as groceries and medications, ensuring materials received were as ordered and immediately stored as appropriate;
Comply with medication protocols and procedures, accurately maintains related records i.e. logging, inventory, counts
Ensure all visitors - guests and official visitors - are identified and logged in;
Ensure official visitors are briefed on assigned clients, receive and log briefing following visit;
Supervise visitors to ensure safety standards are followed;
Search, itemize and issue receipts for any materials or monies dropped off for residents;
Secure cash and valuables as required in the designated locked area;
Issue Daily Parole Allowance, and other allowances/finances to residents as directed;
Immediately inventory, pack and secure property of residents when they leave;
Remain awake and alert on duty to ensure responsiveness to safety and security issues;
Take initiative if next shift staff do not arrive and remain in the House until alternative arrangements are completed
Resident Support Duties
Initiate and establish professional and supportive relationships with residents, interacting with them whenever possible and appropriate;
Supervise the daily activities of the house;
Participate in meal preparation and assist residents to prepare meals;
Participate in housekeeping tasks and assist and supervise residents to do chores;
Escort residents as directed to appointments and programs, ensuring safety;
Evaluate residents for their level of functioning in various life skills, e.g. budgeting, hygiene, cooking etc.
Coach and monitor life skills to increase skill acquisition
Recognize antisocial thinking and behaviours; address them immediately in a constructive and non-threatening manner. Acknowledge and reinforce pro-social thinking and behaviour.
Other Duties
Attend staff meetings and training as directed, this includes Urinalysis Collection duties;
Other duties as assigned by the House Manager or Executive Director
Communications and Interpersonal Skills
Facilitate productive communications while maintaining a professional, pro-social relationship with residents, employees and visitors;
Maintain appropriate boundaries with residents and follow the UCHH Code of Ethics;
Express empathy regarding resident situations as appropriate;
De-escalate potentially volatile situations with residents;
Utilize stages of change and motivational interviewing as tools in interacting with residents;
Effectively communicate pertinent information to co-workers and other agency staff in a timely manner;
Demonstrate collegial relationships with co-workers and other agency staff as an effective team member;
Act respectfully with residents, staff, visitors, neighbours and others;
Maintain confidentiality of resident information and files
Modeling Skills and Knowledge
Display pro-social values, thinking and behaviours;
Display appropriate boundaries and professionalism;
Display courtesy, respect and empathy to residents, co-workers and visitors;
Demonstrate proper food handling, nutritious cooking and personal hygiene;
Assist residents in efforts to problem solve;
Assist clients in mediating and regulating behaviours and emotions;
Demonstrate an understanding of the principles of social learning, criminogenic risks and needs, cognitive-behavioral therapy
Physical Requirements
Regularly required to stand and walk upstairs (3 story building plus basement);
Occasionally must lift and/or move up to 25 lb.
Vision acuity to enable both close and far work, with peripheral vision, depth perception
Be able to work and adjust to varying shifts including overnight shifts while maintaining focus. Shift work can include day, evening, night or weekend shifts, in length up to 12 hours.
Qualifications
Related college/university level education preferred
Related previous job experience
Must be eligible for Enhanced Security Clearance (Federal Government)
Current CPR, First Aid, Non-Violent Crisis Intervention and Applied Suicide Intervention Skills Training (training will be provided if necessary)
Demonstrated people management skills including conflict resolution, good communications skills, ability to problem solve
Ability to cook meals
Ability to work without direct supervision
Must have attention to detail and be able to accurately follow procedures
Ability to work in a stressful environment and to have a non-anxious presence
Reliability and dependability to attend scheduled shifts
