Program Overview
The Enhanced Developing Work Connections program focuses on supporting social inclusion by promoting skill development in the areas of social communication, independence, and pre-vocational skills. Through workshops and volunteering opportunities, participants are supported to find meaningful volunteering and vocational experiences. The Disability Services Department provides Person Directed Planning (PDP) as a member of COMPASS, which is a collaborative of seven community organizations in Toronto with expertise in person-directed planning. Using an individualized approach, skilled facilitators assist people with a developmental disability to develop and implement a plan and help to make connections for the individual to live their dreams and meet their goals. A plan can help and individual to, define a vision of their future life, identify natural abilities and gifts, set goals, make choices, participate in the community, and connect to people who can help them achieve their dreams.

We\xe2\x80\x99re looking for someone who will:

  • Participate in the assessment of client strengths and abilities, and in the development, implementation and goal setting with assigned clients
  • Support clients to engage in the community through recreation and volunteer activities involving travel to off-site locations
  • Ensure a positive, safe, and predictable environment for clients through the establishment and implementation of routines and schedules
  • Research for and creation of program workshops
  • Communicate and liaise with families and other community partners when necessary to coordinate client services
  • Conduct outreach, research, networking, and other initiatives to identify and attract employers who can benefit from placing program clients in volunteer (or employment) positions
  • Contribute and work with job developers across the agency
  • Develop, implement, and maintain client Individualized Support Plan and up-to-date client records
  • Responsible for petty cash used to support client activities
  • Liaise with Developmental Services Ontario regarding the intake process
  • Represent JVS Toronto Disabilities programs at job fairs and other community events
  • Demonstrate the ability to carry out all safety requirements and emergency procedures for clients
  • Assist participants with primary care, such as washroom and lunch monitoring, as needed
  • Be skilled in developing and implementing person directed plans for clients with developmental disabilities
  • Have experience with planning and coordinating supports including family members and other service providers using approved PDP facilitation methods and tools
  • Have familiarity with Developmental Services Toronto and have ability to follow up with reporting and communications
  • Demonstrated ability to assess work and life environments and identify strategies to accommodate and maintain client success
  • Uphold the principles of person-directed planning, enable everyone in the group to participate in the planning meeting, promote a positive outlook towards the person\xe2\x80\x99s dream or vision of the future, ensure the planning process stays focused on the person\xe2\x80\x99s strengths, abilities and what they want, and assist the group to develop clear, measurable, achievable outcomes
Flexible hours / evenings / weekends may be required for this position.

Minimum Qualifications & Experience:
  • College Diploma in Social Services or a related field with 1-2 years of experience working with adults with physical and intellectual disabilities or a High School Diploma with 5-6 years of experience working with adults with physical and intellectual disabilities
  • Effective verbal and written communication skills
  • Strong interpersonal skills; demonstrated ability to work well in a team environment and relate both to individual clients and stakeholders
  • Effective organization and time management skills
  • Demonstrated marketing skills
  • Knowledge of the demands of the private sector
  • Computer literate; the ability to learn systems used by the agency (ie. SalesForce)
  • A clear Police Vulnerable Sector check is required for this position
Our ideal candidate will also have:
  • Up to date CPR and First Aid certification
  • Up to date CPI Certification
